摘要
目的利用灰阶超声造影评价不同级别的脾脏锐器伤,探讨超声造影与不同级别脾损伤之间的关系。方法健康杂种犬6只,麻醉后开腹暴露犬脾脏,使用小型手术月柄分别于脾的脏面和膈面制作不同长度和深度的创缘,形成相对粗糙的病灶。即刻对病灶行二维超声及灰阶超声造影检查。结果在6只实验犬脾脏的脏面和膈面,共制作深度为0.5~6.0 cm不同大小的锐器伤病灶共77处。二维灰阶超声对于深度<3.0 cm的病灶不能良好显示,病灶显示率为24%(12/50);而灰阶超声造影能清晰显示所有的病灶,显示率为100%。结论灰阶超声造影可清晰显示脾损伤的病灶位置和范围,并可部分显示病灶内的活动性出血,显著提高了超声对不同程度损伤病灶诊断的准确性。
bjective To evaluate the different degree of spleen injury with gray scale contrast - enhanced ultrasonography (CEUS), and to discuss the relatiomhip of CEUS with different spleen injury in animals. Methods Injuries with different length and depth were created in the spleen of 6 healthy mongrel dogs after the animals were anesthetized and laparotomized. Two-dimensional ultrasonography (2D-US) and CEUS were performed to observe the characteristics of traumatic region. Results Among 77 injuries, 21)- US was not able to detect the injuries with the depth less than 3.0 cm, the detection rate of 2D-US was about 24% ( 12/50). After injecting contrast agent, all traumatic regions could be detected with a rate of 100%. Conclusion CEUS could clearly detect the position and size of traumatic region and some active hemorrhage in traumatic spleen, which shows high accuracy rate in detecting different degree of splenic injuries.
