经阴道彩色多普勒超声对卵巢黄体血肿的诊断价值

Clinical value of transvaginal color Doppler sonography in the diagnosis of ovarian luteal hematoma

目的探讨经阴道彩色多普勒超声(TVCDS)对卵巢黄体血肿的诊断价值。方法采用TVCDS检查68例卵巢黄体血肿,观察病变的边界,囊壁及内部分隔带的厚度,内部回声,CDFI检查病变边缘及内部的血流速度,并应用频谱多普勒测量病变边缘和内部动脉阻力指数(RI)、搏动指数(PI)。超声检查结果与手术病理结果和临床随访结果进行对照。结果68例卵巢黄体血肿中囊性血肿59例,囊实混合性血肿5例,类实体性血肿4例,CDFI显示80.9% (55/68)的卵巢黄体血肿周边见环状或半环状血流信号。TVCDS诊断卵巢黄体血肿的符合率为94.1%(64/68)。结论TVCDS对卵巢黄体血肿的诊断有重要的临床应用价值。

Objective To investigate the clinical value of transvaginal color Doppler sonography （TVCDS） in the diagnosis of ovarian luteal hematoma （OLH）. Methods Sixty - eight cases of OLH were examined with TVCDS. The boundary of lesions, thickness of cyst wall and inner compartment as well as internal echoes were observed. The blood flow in periphery and interior of lesions was detected with color Doppler flow imaging （CDFI） and the value of RI and PI were measured with pulse Doppler. The results of TVCDS were compared with those of pathology after operation and clinic follow - up. Results Sixty - eight cases were divided into three types： cyst pattern （59 cases）,mixed solid and cystic pattern （5 cases） and solid like pattern （4 cases）. CDFI showed ring and half- ring color blood flow signals in the cystic- solid mixed periphery of 55 cases （80.9%）. The diagnostic rate of TVCDS for OLH was 94.1% （64/68）. Conclusion TVCDS has important clinical value in the diagnosis of OLH.
