老年住院病人循环系统疾病住院费用分析

ANALYSIS ON EXPENSES OF HOSPITALIZATION OF OLD INPATIENTS WITH CIRCULATORY DISEASE

[目的]了解徐州市老年住院病人循环系统疾病医疗费用情况,为政府制定合理的医疗费用提供决策依据。[方法]对收集的老年住院病人循环系统疾病资料按国际疾病分类(ICD-10)为标准进行诊疗状况及住院费用分析。[结果]1999～2004年徐州市老年循环系统疾病患者住院人数、人均费用中位数和日均费用中位数逐年增加,而住院天数中位数逐年减少。不同年龄、性别、职业、预后的住院天数中位数、人均费用中位数、日均费用中位数有明显差异。循环系统疾病不同病种医疗费用快速增长,主要表现在人均费用中位数在前3位的分别为脑出血,脑梗塞,脑动脉硬化,差异有统计学意义(P<0.01),日均费用中位数在前3位的分别为脑出血,脑梗塞,肺心病。差异有统计学意义(P<0.01)。[结论]随着人口老龄化快速发展,老年住院病人循环系统疾病不仅影响患者的生命和生活质量,也给社会尤其是患者的家庭带来沉重的经济负担。降低老年住院病人循环系统疾病医疗费用已成为当前的热点,政府、社会应给予更多的关注。

[Objective] To understand the medical expenses of old inpatients with circulatory disease in Xuzhou, to provide bases for government making reasonable medical costs. [Methods] The data were collected from old inpatients with circulatory disease, which were analyzed by ICD-10. [ Results] The number of old inpatients, medians of per capita expenses and expenses at every day of old inpatients with circulatory systerm disease increased, but median of hospitalization days decreased annually from 1999 to 2004 in Xuzho,. Median of hospitalization days among age, gender, occupation and prognosis, medians of per capita expenses and expenses at every day were obviously different. Circulatory disease increased rapidly, which mainly reflected from the first three medians of per capita expenses were cerebral hemorrhage, cerebral infarction and cerebral atherosclerosis. The first three medians of expenses at everyday were cerebral hemorrhage, cerebral infarction and pulmonary heart disease. The differences had statistical significances （P〈 0.01 ）. [Conclusion] With the rapid development of population aging, old inpatients＇ circulatory disease not only influence qualities of life, but also bring heavy economy burden to patients＇ family. Decreasing medical expenses of old inpatients with circulatory disease has become the current hot issue, and government and society should give more concern.
