^(18)F-FDG符合线路SPECT显像诊断缺铁性贫血的肿瘤病因

摘要 目的探讨18氟-氟代脱氧葡萄糖(18F-fluorodeoxyglucose,18F-FDG)符合线路SPECT显像在筛查缺铁性贫血患者恶性肿瘤病因中的价值。方法对53例缺铁性贫血患者进行18F-FDG符合线路SPECT显像检查。静注18F-FDG 5~8 mCi,45 min后开始扫描,进行X-ray断层采集,经计算机重建获得全身横断面、冠状面和矢状面断层影像。结果53例缺铁性贫血最后确定的病因为胃肠道恶性肿瘤31例,非肿瘤性疾病22例。符合线路显像的敏感性为90.3%(28/31),特异性81.8%(18/22),准确率86.8%(46/53),阳性预测值87.5%(28/32),阴性预测值85.7%(18/21)。结论18F-FDG符合线路SPECT显像作为筛查诊断手段,可以初步确定潜在恶性肿瘤的有无及其所在部位,准确率较高。 Objective To study the value of 18↑F-fluorodeoxyglucose (18 ↑F-FDG) SPECT with coincidence image in detection of malignant causes of iron deficiency anemia. Methods 18↑ F-FDG SPECT image was carried out in 53 patients with iron deficiency anemia. Transaxial, coronal and sagittal section image of the body were reconstructed by computer based on X-ray scan 45 rain after intravenous injection of 18↑F-FDG5- 8 mCi. Resuits In 53 patients, 31 cases of iron deficiency anemia were caused by gastroenteric malignancies and 22 cases by non-malignant diseases. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18↑ F-FDG SPECT image were 90.3 % (28/31), 81.8 % (18/22), 86.8 % (46/53), 87.5 % (28/ 32) and 85.7 % ( 18/21 ) ,respectively. Conclusion 18↑ F-FDG SPECT image can be used as screening methods for detecting malignancies underlying iron deficiency anemia with fairly good accuracy.
