彩超诊断室间隔缺损合并畸形的漏、误诊分析

Analysis of Missed diagnosis and Misdiagnosis of Anomalies Accompanied with Ventricular Septal Defect by Using Echocardiography and Color Doppler Flow Imaging

【目的】探讨彩色多普勒超声心动图诊断室间隔缺损(室缺)合并畸形的漏、误诊原因。【方法】将手术证实的室缺合并畸形与超声检查进行比较,分析其敏感度、漏诊率及误诊率。【结果】超声心动图检出室缺合并畸形的敏感度为64.1%,漏诊率为35.9%,误诊率为3.4%。【结论】室缺合并卵圆孔未闭、右室流出道狭窄及双腔右心室时容易漏诊,注意全面扫查及改善检查技巧有助于提高室缺合并畸形的诊断和减少漏、误诊。

[Objective]To study the cause of missed diagnosis and misdiagnosis of anomalies accompanied with ventricular septal defect （VSD） by using echocardiography and color Doppler flow imaging, [Methods] The anomalies accompanied with VSD confirmed by operation were compared by using echocardiography, and echocardiographic sensitivity, and the rate of missed diagnosis and misdiagnosis were analyzed. [Results]The sensitivity, the rate of missed diagnosis and misdiagnosis of the anomalies accompanied with VSD detected by echocardiography were 64.1%,35, 9% and 3, 4 %respectively. [Conclusion] Patent foramen ovale, right ventricular outflow tract stenosis and double-chambered right ventricular accompanied with VSD are likely to be missed by echocardiography, Systematic scanning and improvement of scan technique would increase the rate of diagnosis of anomalies accompanied with VSD, and decrease missed diagnosis and misdiagnosis.
