摘要
目的:探讨定位模板结合颌骨曲面(PR)及横断断层摄影(LT)在种植受术中的临床应用价值。方法:对20例种植牙患者术前制作透明塑料模板,在拟种植位点放入直径为5mm的钢球,带入患者口内进行颌骨的曲面及横断断层摄影,根据所得图像测出颌骨受植区的深度、宽度及厚度等相关数值,以指导临床选择合适型号的种植体,种植后立即重复摄影,及时纠正种植体植入中存在的角度和深度等问题。结果:60颗种植体中,1例种植体角度偏离并穿破颌骨皮质,1例种植体邻近邻牙牙根,重新植入后效果满意,其中22颗前牙采用了GBR植骨术。术后半年随访所有种植牙体形态位置均无异常改变。结论:种植导板与PR及LT相结合运用,能让医生在术前对受植区的颌骨有全方位的认识,对种植牙手术计划的制定与实施,提高种植牙体的成功率具有重要的临床应用价值。
Objective: To evaluate the clinical application of panoramic radiography (PR) and linear tomography (LT) with templates in the operation planning of dental implants. Methods:The transparent and plastic templates were made. A hole was drilled at the position of implantation,and it was filled with radioopaque gutta. PR and LT were undertaken in 20 patients. The implant depth,width and thickness of the recipient region of the jaw bone before operation were precisely measured in order to guide the selection of appropriate size of implanted denture and treatment planning. PR and LT were performed immediately after denture implant and, the effect of implantation was evaluated for the correction of the depth or angularity of the implant. Results:Of these 60 implanted dentures,one implanted denture showed marked deviation and the penetrated jawbone cortex ,one implanted denture was closed to the root of near tooth but the satisfactory effect was obtained after reimplantation. GBR were used for 22 anterior teeth. All cases were followed up for 6 months and no serious complications were found. Conclusion :The technique of PR and LT combined with templates can help doctors in the omnibearing comprehension of the jaws before operation,which plays an important role in the operation planning and the success rate of implantation can be improved.
