Expression and clinical significance of decoy receptor 3 protein in human gastric carcinoma

摘要 目的探讨诱捕受体3(decoy receptor3,DcR3)蛋白在胃癌组织中的表达及其与各临床病理因素的关系,评价DcR3在胃癌预后判断中的意义。方法采用免疫组化SP法检测98例胃癌组织和56例癌旁正常胃组织中DcR3的表达,分析其与多种临床病理因素及预后之间的关系。结果胃癌组织中DcR3阳性表达率为55.1%(54/98),其中高分化组阳性表达率为39.0%,明显低于低分化组的79.5%(P=0.00),癌旁正常胃组织中无阳性表达;DcR3表达与胃癌淋巴结转移和TNM分期呈显著正相关(P=0.00),与患者性别、年龄、肿瘤部位及浸润深度等临床因素无相关性(P>0.05);DcR3阴性表达组的5年生存率为61.3%,明显高于阳性表达组的29.6%(P=0.002)。多因素分析显示,DcR3表达不是胃癌术后独立的预后因素。结论DcR3在胃癌组织中具有较高的表达率,其表达与肿瘤分化程度、淋巴结转移及TNM分期密切相关。DcR3高表达提示预后不良。 Objective To investigate the relationship between the expression of decoy receptor 3 (DcR3) protein and the clinicopathological parameters in human gastric carcinoma, and evaluate its clinical significance in predicating the prognosis. Methods DcR3 protein expression was examined by immunohistochemistry S-P staining method in 98 cases of gastric carcinoma tissues and 56 cases of nomal tissues adjacent to tumor. Multiple clinicopathological factors and prognosis were retrospectively analyzed according to their relation with the expression of DcR3. Results DcR3 protein expression was detected in 54 out of 98 gastric carcinoma cases with a positive rate of 55.1%. No DcR3 protein expression was detected in the 56 cases of nomal tissues adjacent to tumor. DcR3 protein expression rate in highly differentiated tumors was significantly lower than that in poorly differentiated (39.0% vs. 79.5 M, P=0.00). The DcR3 protein expression level was significantly positively correlated with lymph node metastasis and TNM staging (P=0.00), but there was no significant difference in DcR3 protein expression and other clinicopathological features such as sex, age, tumor status and invasion depth (P〉0.05). The five-year survival rate of patients was significantly higher in DcR3 negative group than in DcR3 positive group (61.3 ~ vs. 29.6~, P = 0. 002). Multivariate analysis with the Cox proportional hazards modal showed that DcR3 protein expression was not independent prognostic factor for the patients with gastric carcinomas after a resection. Conclusion DcR3 protein expression can be highly found in human gastric carcinoma. The DcR3 protein expression is significantly associated with different degrees of differentiation, lymph node metastasis and TNM staging. DcR3, its strong expression being correlaion with poor prognosis of gastric cancer patients, is not an independent predictor for human gastric carcinoma.
