血清CA125和CA19-9水平对子宫内膜异位症诊断价值的探讨

Study on the Diagnostic Value of Determination of Changes of Serum CA125 and CA19-9 Levels in Patients with Endometriosis

目的:探讨血清CA125和血清CA19-9水平对子宫内膜异位症的诊断价值。方法:应用放射免疫分析测定45例子宫内膜异位症患者血清CA125和CA19-9水平,并和40例正常健康人作比较。结果:子宫内膜异位症组及对照组血清CA125检测值及阳性率分别为(89.63±30.16)U/ml、77.78%和(16.26±6.37)U/ml、7.50%;子宫内膜异位症及对照组血清CA19-9检测值及阳性率分别为(95.62±43.67)U/ml、62.22%和(20.26±8.25)U/ml、10.00%;子宫内膜异位症患者血清CA125和CA19-9检测值显著高于对照组(P<0.001);子宫内膜异位症患者血清CA125和CA19-9阳性率显著高于对照组(P<0.001);子宫内膜异位症血清CA125敏感性及特异性分别为77.78%及92.50%;血清CA19-9敏感性及特异性分别为62.22%及90.00%;血清CA125及CA19-9联检的敏感性及特异性分别为57.78%及100%;子宫内膜异位症(Ⅰ、Ⅱ)期与(Ⅲ、Ⅳ期)血清CA125和CA19-9检测值差异显著(P<0.001);血清CA125阳性率无显著差异(P>0.05);血清CA19-9阳性率差异显著(P<0.05)。结论:测定子宫内膜异位症患者血清CA125和CA19-9水平对子宫内膜异位症有一定的诊断价值,联检CA125和CA19-9水平更有实用价值。

Objective To explore the diagnostic value of determination of changes of serum CA125 and CA19 - 9 levels in patients with endometriosis. Methods Serum CA125 and CA19 - 9 levels were determined with RIA in 45 patients with endometriosis and 40 controls. Results Serum levels of CA125 and CA19 - 9 in patients with endometriosis （ 89.63 ± 30.16u/ml and 95.62 ± 43. 67u/ml respectively） were significantly higher than those （ 16.26 ± 6.37u/ml and 20.26 ± 8.25u/ml respectively） in controls （ P 〈 0. 001 ）. Sensitivity of CA125 for diagnosis of endometriosis was 77.78% and that of CA19 - 9 was 62.22% , while the specificity was 92.50% and 90.00% respectively. If we only took the patients with both markers elevated as being diagnostically positive, the sensitivity would naturally be lower （57.78%） but the specificitiy would be 100%. Levels of the markers were significantly higher in stage Ⅲ, Ⅳ patients than those in patients with stage Ⅰ , Ⅱdisease （P 〈 0.01 ）. Conclusion Combined determination of serum CA125 and CA19 -9 levels were of highly specific diagnostic value in patients with endometriosis.
