高热 咳嗽 气喘伴皮下气肿

A Patient with High Fever, Cough, Breathlessness and Subcutaneous Emphysema

患者男性,16岁,主因"发热1月余,咳嗽、气喘伴颈部淋巴结肿大10天"入院,曾于外院行左锁骨上淋巴结活检,病理报告为"T-细胞性淋巴瘤",入院后患者因皮下气肿及憋气而行纵隔气肿引流术,无明显好转,右后肋肺部肿物穿刺术,病理示(右肺)肺母细胞瘤。患者皮下气肿持续加重,发热、憋气等症状无缓解。应用IFO2gd1-3+DDP50mgd1-3+E-ADM80mgd1化疗一周期,患者病情明显好转出院。肺母细胞瘤是一种罕见的肺原发肿瘤。此病例对化疗敏感,治疗效果好。

A 16 year old male patient was admitted due to having a fever for more than one month accompanied by euugh, breathlessness and cervical lymphadeneetasis for 10 days. He was initially diagnosed with T-cell lymphoma and the left Supraclavieular lymph nodes were removed. After admission, subcutaneous emphysema and hreathlessness heeame severe. Mediastinal emphysenta drainage briught no relief. Lung tumor centesis was performed and the tumor was identified as pulmonary, blastoma （PB） by pathology. The symptoms became progressively more serious. After one cyele of chemotherapy, the patient was discharged with remarkable improvement. The regimen was administered as follows： IFO 2g Ⅳ （from 1st to 3rd clay）, DDP 50 mg Ⅳ （from Ist to 3rd day）, and E-ADM 80 mg Ⅳ （on the 1st day）. Pulmonary hlastoma（PB） is a rare primary malignant lung neoplasm. In this ease. chemotherapy aehieved satisfaelory therapeutic effects.
