盆底痉挛综合征的盆底肌电图表现分析(附67例报告) 被引量:2

摘要 为了探讨盆底痉挛综合征患者的盆底肌肉肌电变化,对确诊为盆底痉挛综合征患者67例进行盆底肌电图结果分析。盆底痉挛综合征患者的盆底肌电图显著表现为反向收缩。静息电位、最大收缩相异常随并发症增多而增多;动作电位的整体趋势是波幅和时限降低,但亦有少部分升高。反向收缩是盆底肌电图诊断盆底痉挛综合征的重要指标,盆底肌反常收缩是盆底横纹肌整体反射性弛缓功能失常,不应割裂“盆底痉挛综合征”和“耻骨直肠肌肥厚综合征”;盆底痉挛综合征患者的盆底肌在主动收缩时肌张力不仅不升高,反而有所降低;随着便秘症状的加重,致病原因的增多,盆底肌肉逐渐从功能性异常向器质性异常转变,从一定意义上讲,功能性便秘应该只包括“痉挛性”便秘,因为“松弛性”便秘随着检查手段的提高,均会发现其器质性改变。 Analysis of pelvic floor electromyography(PFE) of 67 patients with confirmed spastic pelvic floor syndrome(SPFS) was made in an attempt to study their myoelectric changes. As results of this study, the notable change of the PFE of the patients with SPFS was reverse contraction,abnormal resting potential and maximum contraction phase became frequent as the complications became increased in number, while the action potential became lower in general in voltage and amplitude(but,it became higher in small amount). It is concluded that reverse contraction is the important diagnostic index for SPFS, that the abnormal contraction of the pelvic floor muscle reflects the reflex relexation of the pelvic floor striated muscles as a whole,so the concepts of SPFS puborectal hypertrophic syndromecould not be separated,that the muscular tention does not increase (becomes decreased in fact) when the pelvic floor muscle contracts,and that the pelvic floor muscle becomes abnormal from functional abnormality to organical abnormality. By the certain meaning,functional constipation should really mean alone "spastic" constipation, while eventually flaccid constipation could be examined having organic lesion with the assay tecnology is improved.
