苦参碱注射液治疗婴儿巨细胞病毒性肝炎疗效观察

The treatment of infants' cytomegalovirus hepatitis with Matrlne injection

目的:评价苦参碱注射液治疗婴儿巨细胞病毒(CMV)肝炎的疗效。方法:将42例巨细胞病毒肝炎患儿随机分为治疗组(n=22)与对照组(n=20)。两组均以常规保肝降酶治疗为基础。治疗组用苦参碱注射液治疗共2～3个疗程。对照组用更昔洛韦治疗共2～3个疗程。观察两组临床效果。结果:①治疗组与对照组治疗6周后CMV-DNA转阴率分别为78.8%、85.3%,两组无显著性差异(P>0.05)。②两组患儿治疗4周后肝功能及胆红素无显著性差异(P>0.05);治疗组治疗前与治疗4周后谷丙转氨酶、总胆红素、直接胆红素、间接胆红素比较有非常显著性差异(P<0.01);而与胆汁酸在6周后比较有显著性差异(P<0.05)。③两组患儿治疗6周后疗效比较无显著性差异(P>0.05)。结论:苦参碱注射液具有与更昔洛韦相似的抗HCMV感染的作用,其效果虽弱于更昔洛韦,但与之相近,而且有保护肝细胞等作用。因此治疗婴儿巨细胞病毒性肝炎疗效确切,可推广应用。

Objective： To observe the treatment of infants＇Cytomegalovirus hepatitis with Matrlne injection. Methods： 42 cases of CMV hepatitis were randomly divided into tow groups： the treated group （n=22） and the control group （n =20） . The treated group with Matrlne （20-25 mg/d） once daily for 14 days. After 7-10 days with Matrlne （20-25 mg/d ） once daily for 5 days, then repeat 2-3 time. The control group with Ganciclovir 5 md/kg·d twice daily for 14 days. After 7-10 days with Ganeiclovir 5md/kg·d once daily for 5 days , then repeat 2-3 time. Results：①There were not significant differences in CMV-DNA negative between the two groups after 6 weeks （P〉 0.05）. ②There were not significant differences in ALT, TBIL between the two groups after 4 weeks （P〉0.05） . There were significant differences in TBA between the treated groups after 6 weeks （P〈0.05） and there were very significant differences in ALT, TBIL, DBIL, IBIL between the treated groups after 4 weeks （P〈0.01）.③There were not significant differences in Treatment between the two groups after 6 weeks （P 〉0. 05） . Conclusion： The effect treated HCMV with Matrlne injection similar as Ganciclovir. And it can protect scathing liver cell , accelerate jautice faraway.
