远程胎儿监护在高危妊娠中的临床分析与应用

The clinical analysis and application of long distance fetal heart rate monitoring on the high risk pregnancy

目的：探讨胎心监护仪在高危妊娠监测中的临床应用及意义。方法：选择孕期32～40周且确诊为高危妊娠的孕妇220例，随机分为研究组112例，对照组108例。结果：研究组无应激试验异常检出率为40．18％，对照组为20．37％，两者比较有显著性差异（P〈0．05）；新生儿窒息率研究组为4．5％，对照组为8．3％，两者比较有显著性差异（P〈0．05）；研究组顺产率和剖宫产率分别为66．96％和33．04％，对照组分别为63．89％和36．11％，两组比较，对照组剖宫产率稍高，但差异无显著性（P〉0．05）。结论：应用远程胎儿监护对高危妊娠进行监测，可降低新生儿窒息等围产儿预后不良的发生，为高危妊娠孕妇的自我监护和高危妊娠管理开辟了一条有效、实用、可行的途径。

Objective： To investigate the clinical application and significance of long distance fetal heart rate monitoring on the high risk pregnancy. Methods： 220 pregnant women with duration of pregnancy of 32 - 40 week being high risk pregnancy in the clinic obstetrics service were selected and divided into two groups： 112 pregnant women in observing group, 108 pregnant women in control group. Results： The anomal - detection rate of observing group was 40. 18%. The rate was significantly different from that of control group, 20. 37% （ P 〈 0. 05 ） . The rate of apnoea neonatorum in observing group was 4. 5% , control group was 8. 3%. There was significant difference （ P 〈 0. 05 ） . In observing group, the rates of spontaneous delivery and uterine - incision delivery were 66. 96% and 33.04% respectively, and the control group were 63.89% and 36. 11%. There was no significant difference between the rate of uterine - incision delivery in observing group and in control group （ P 〉 0. 05 ） . Conclusion： The rates of apnoea neonatorum and the other things such as perinatal dys- property combe are decreased by utilizing fetal heart rate monitoring on the high risk pregnancy. An effective , practical and feasible way can be provided for the high risk pregnant woman by long distance fetal heart rate monitoring.
