颅内出血早产儿血小板参数变化的临床意义 被引量:3

The clinical significance of changes in platelet parameters with periventricular-intraventricular hemorrhages in premature infants

摘要 目的:研究颅内出血早产儿血小板参数的变化,探讨其临床意义。方法:检测68例脑室周围-脑室内出血的早产儿血小板参数变化,并与同期无颅内出血的早产儿45例和正常足月新生儿27例的各项指标进行比较。结果:血小板计数分别为(292.96±46.91)×109/L、(220.61±35.01)×109/L和(188.85±51.62)×109/L,血小板压积分别为(0.232±0.07)%、(0.188±0.06)%和(0.126±0.04)%,提示脑室周围-脑室内出血患儿血小板计数和血小板压积显著降低(F=68.66,P<0.001);而血小板平均容积与血小板分布宽度差异不显著(P>0.05)。结论:血小板参数的异常变化可能参与了早产儿脑室周围-脑室内出血的发病机制,要重视对早产儿凝血状态的监测,以减少其发生。 Objective: To study clinical significance of changes in parameters of platelet with periventricular intraventricular hemorrhages (PIVH) in premature infants. Methods: The parameters of platelet including platelet count (PLT, × 10^9 L), thrombocytocrit (PCT,%), mean plate volume (MPV, fl) and platclet distribution width (PDW,%) were examined respectively in 68 premature infants with PIVH and 45 premature infants without PIVH and 27 healthy terminfants. Results: As compared with terminfants and premature infants without VII , PLT and PCT of infants with PIVH were lower singniilcantly ( PLT: 292. 96 ± 46. 91 × 10^9 L, 220. 61 ± 35. 01× 10^9L and 188. 85 ± 51.62 × 10^9 L, respectivelyPCT: 0. 232 ± 0. 07%, 0. 188 ± 0. 06% and 0. 126 ± 0. 04%. F = 68. 66, P 〈 0. 001 ), while MPV and PDW were similarinall groups ( P 〉 0. 05 ). Conclusion: The changes inparameters of platelet may involve the pathogenesis of PIVH in premature infants , and more attention should be paid inmonitoring of coagulation status of premature infants so as to reduce the incidence of PIVH.
