Clinical Application of Resection of Lower Lobe and Lingular Bronchus for Left Lung Bronchiectasis

目的探讨下叶切除加舌段支气管剔除治疗左肺支气管扩张的临床应用。方法回顾分析近两年半来施行的18例此种手术,总结支扩的病因学、诊断治疗以及支气管剔除术的理论基础、禁忌适应症、操作要点。结果无死亡病例,1例术后并发肺不张,1例术后持续漏气,1例术后3个月左上叶复发,余病例术后恢复良好,随访3～28个月,咳脓痰、咯血症状消失,临床效果满意。结论该术式损伤小、出血少,手术效果良好,是一种理想的治疗手段。

Objective To explore clinical application of resection of lower lobe and lingular bronchus for left lung bronchiectasis. Methods 18 patients with the operation were analysed retrospectively. And sum up etiology, diagnosis, treatment of bronchiectasis, and rationale, contraindication, indication, and the operation key points of bronchus reject operation. Results There was no death case, 1 case pulmonary atelectasis, 1 case air leakage persistently, 1 case left and upper lung relapse after 3 months, the other cases recovered well. Vomica and hemoptysis disappeared after 3- 28 months, the clinical effect was good. Conclusion With little injury, few hemorrhage and good clinical effect, the operation is a good therapeutic way.
