糖尿病患者微量白蛋白尿的临床分析

Clinical Analysis of Microalbuminuria in Diabetes Patients

目的探讨糖尿病患者微量白蛋白尿与高血压、脂代谢异常之间的关系。方法将连续入选的糖尿病住院患者分为高血压组及非高血压组,检测血甘油三酯、高密度脂蛋白、空腹血糖、2h血糖、糖化血红蛋白、空腹胰岛素、C肽、血清微球蛋白、尿微球蛋白、24h尿白蛋白排泄率。结果高血压组的UAER为(32.4±25.06)mg/24h,明显高于非高血压组(22.90±18.85)mg/24h(P<0.01),两组MAU检出率分别为43.93%、29.17%。MAU患者年龄、FPG、2hPG、HbA1c、血清β2-MG、尿β2-MG、UAER、TG明显高于正常白蛋白尿患者,两组相比差异具有统计学意义,且UAER随着患者代谢异常因素的增加而增高。结论糖尿病患者UAER增高与高血压、高TG、低HDL-C密切相关,有高危因素患者应尽早进行MAU的筛查。

Objective To discuss the relationship between microalbuminuria and hypertension and abnormal lipid metabolism. Methods The patients were divided into two groups, hypertension groups and non-hypertension groups. The serum triglyceride, high-density lipoprotein, fasting plasma glucose, 2 hours plasma glucose,glycosylated hemoglobin, fasting serum insulin, C-peptide, serum β2-microglobulin, urine β2-microglobulin, urine albumin excretion rate of 24 hours were detected. Results The UAER was higher in hypertension groups than in non-hypertension groups（P〈O.O1）, MAU was 43.93% and 29.17 respectively. The age, FPG, 2hPG, HbAlc, serum β2- MG, urine β2- MG, UAER, TG were higher than normal albuminuria patients. Conclusion The increase of UAER in dibetes patients is closely related to hypertension, high TG, low HDL-C. The patients of high risk factors should screen as soon as possible.
