Factors affecting incisal inclination during non-extraction treatment: A multivariate analysis

摘要 目的探讨影响非拔牙病例矫治前后前牙转矩改变的因素。方法选择轻度拥挤、采用不拔牙矫治、整平排齐牙列的临床病例为研究对象,对139个样本的头颅侧位片、模型和病历中得到的16个因素进行多因素分析。结果影响矫治前后中切牙转矩改变的因素有5个,按影响由大到小排列为中切牙与功能性平面的交角、拥挤度、SN与颌骨的成角、颌间牵引的使用和结束弓丝的材质和尺寸;影响矫治前后中切牙转动中心相对位置的因素有3个,按影响由大到小排列为:矫治前后牙弓前部深度的改变量、矫治前牙列的拥挤度和颌骨与SN的成角。结论中切牙与功能性平面的交角、拥挤度、SN与颌骨的成角、颌间牵引的使用和结束弓丝的材质和尺寸可影响非拔牙病例矫治前后前牙转矩的改变。 Objective To investigate the factors affecting incisal inclination during non -extraction treatment. Methods One hundred and thirty nine non - extraction patients with 0 - 4mm crowding were selected. The relationship between incisal inclination change and 16 potential influencing factors (from cepholometrics , model analysis and treatment records) was analyzed. Results Among the 16 potential factors, five factors (the angle between incisal axis and functional occlusal plane, crowding, the angle between SN and maxilla or mandible, intermaxillary elastics and the material/size of the finishing wire) affected the incisal inclination and three factors ( the change of anterior arch depth, crowding and the angle between SN and maxilla or mandible) influenced the position of the rotation center of the incisor. Conclusion The angle between incisal axis and functional occlusal plane, crowding, the angle between SN and maxilla or mandible, intermaxillary elastics and material/size of the finishing wire might play roles in the incisal inclination change during non - extraction treatment.
