老年患者颅内动脉狭窄的危险因素分析 被引量:2

摘要 目的探讨老年患者颅内动脉狭窄的危险因素。方法对144例老年患者行头颅磁共振血管造影术(MRA)检测,将颅内动脉狭窄者29例为狭窄组,其余125例为非狭窄组,比较两组间2型糖尿病、高血压的患病情况,血压,空腹和餐后2h血糖,糖化血红蛋白(HbA1c),C肽,尿白蛋白肌酐比(Alb/Cr),血脂水平,分析颅内动脉狭窄的危险因素。结果2型糖尿病患者颅内动脉狭窄明显增加(P=0.002),糖尿病合并高血压患者的危险最高,狭窄组和非狭窄组患者的TG、HbA1c、空腹血糖、餐后2h血糖、尿Alb/Cr的分布差异有统计学意义,logistic回归分析,HbA1c和尿Alb/Cr是颅内动脉狭窄独立的危险因素。结论老年患者颅内动脉狭窄与糖、脂代谢紊乱相关。 Objective To investigate the risk factors for intracranial artery stenosis in Chinese old patients. Methods Magnetic resonance angiography(MRA) was performed,and blood pressure, blood lipid, HbAlc,urine Alb/Cr, fasting and postcible blood glucose and C peptide were determined in 144 old Chinese patients who were divided into the intracranial artery stenosis and nonstenosis groups. Results The rate of intracranial artery stenosis was higher in diabetic patients than in non-diabetic ones,and the highest was in those with both diabetes and hypertension. Patients with intracranial artery stenosis had significant increase in triglyceride, HbAlc, fasting blood glucose,postcible blood glucose and urine Alb/Cr. By logistic regression analysis, HbAlc and urine Alb/Cr were found to be the independent predictors for intracranial artery stenosis. Conclusion It suggests that metabolic disorder of glucose and lipid should be related to intracranial artery stenosis in old patients. HbAlc and urine Alb/Cr should be the risk factors for intracranial artery stenosis.
