2例帆状胎盘前置血管破裂患者的护理

目的总结帆状胎盘前置血管破裂孕妇的护理要点。方法给予患者安胎、抗感染、加强营养等治疗并严密观察母婴变化,积极配合抢救。结果2例均为早产,1例行剖宫术,新生儿体重为2 200 g,Apgar评分为1 min 6分、10min 8分;另1例为自然产,新生儿体重为1 480 g,新生儿Apgar评分为1 min 5分、5 min 8分、10 min 10分,均需行气管插管。结论在孕期期间,应重视产前检查,鉴别孕期出血的特征及加强高危孕妇健康教育等,以协助早期诊断和治疗;分娩时助产士应掌握该病临床特征,严密监护,协助围产儿抢救,提高抢救成功率。

Objective To summarize the key points of caring the patients with rupture of vessel previa in velamentous placenta. Methods The patients confirmed with rupture of vessel previa in velamentous placenta were treated with anti-abortion, and closely observed about their changes in both mother and fetus. Active cooperation during saving was conducted. Results The two cases both gave premature birth. One was delivered by caesarean section, weighted 2 200 g and graded 6 at lmin, 8 at 10min by Apgar, and the other by spontaneous labor, weighed 1 480 g, graded 5 at lmin, 8 at 5min and 10 at 10min. Both received tracheal intubation. Conclusions During the pregnancy, nurses should pay attention to vaginal bleeding and focus on post-rupture observation and attach importance to health education to assist early diagnosis and treatment. At delivery, the maternity nurses should understand the clinic characteristics of the disease, cooperatively observe them, assist the saving of fetus, which is helpful for the increase of success rate.
