摘要
目的探讨防止胸腰椎骨折后路手术椎弓根钉系统内固定的失效及内固定物取出后伤病椎高度的丢失的问题。方法使用椎弓根钉系统撑开伤病椎,恢复原节段椎体高度或稍高于原高度(1/5-1/4),手术在X线机下操作、无阻力下注入VERTEBROLPLASTICTM脊柱椎体成形术专用骨水泥。结果8例病人术后无神经症状加重或出现新的神经病状,无内固定松动、断钉、断棒现象;随访4-21个月,伤椎前、中柱的高度随访时同术后无明显丢失现象。结论椎弓根螺钉系统固定加椎体成形术治疗胸腰椎骨折是一种安全有效的治疗方法。
Objective To evaluate the problems,which including how to eliminate the uneffect of transpedicular screw system internal fixation through the posterior approach to treat the thoracolumbar junction vertebral body compression frae -ture, and the height loss of the injured vertebral body after operation of tran spedieular screw system internal fixation removement. Methods Firstly,we use the transpedicular screw system to amply the compressed vertebral body,which can restort the height of the injured vertebral body,or add the height up to about 1/5 - 1/4 more compared with the preheight;then we freely inject the bone cement into the injured vertebral body with the help of C - shape X - ray maehine. Results 8 patients were trea- ted,no complication occurred,such as new nerve injury symptom occured, and the nerve injtury symptom became worse, and the srew system loosen or broke in;Followed up 4 -21 months postoperative, the anterior and middle column height of the injured vertebral body has no additional loss again. Conclusions Transpedicular screw system combined with the vertebroplasty that was used to treat thoraeolumbar vertebral body fi'aeture is safe and efficacy.
