双水平气道正压通气治疗重叠综合征伴呼吸衰竭25例疗效分析

Treatment of senile overlap syndrome complicated with respiratory failure by bi-level positive airway pressure ventilation

目的探讨双水平气道正压通气（Bi—PAP）呼吸机治疗重叠综合征伴呼吸衰竭患者的疗效。方法25例患重叠综合征伴呼吸衰竭的老年患者，在常规治疗基础上，使用Bi—PAP呼吸机治疗，观察治疗前后动脉血气指标变化及睡眠呼吸暂停低通气指数（AHI）变化。结果25例患者使用Bi—PAP呼吸机治疗后，20例（80．0％）血气指标明显改善，AHI指数明显纠正。结论使用Bi—PAP呼吸机治疗重叠综合征伴呼吸衰竭，可有效地减轻患者上气道阻塞，消除低氧血症，降低患者的反复感染率，显著改善预后。

Objective To investigate the therapeutic effect of Bi-level positive airway pressure （Bi- PAP） ventilation in treating senile overlap syndrome complicated with respiratory failure. Methods Twentyfive patients were treated by Bi-PAP ventilation simultaneously after routine treatment. Arterial blood gas and apnea hypopnea index（AHI） were analyzed. Results Indicators of arterial blood gas analysis were improved significantly in 20 among 25 patients received Bi-PAP ventilation, and AHI were corrected significantly. Conclusions Bi-PAP presented effectiveness in treating senile overlap syndrome complicated with respiratory failure, which could alleviate the upper airway obstruction and eliminate hypoxemia of patients and reduce the repeated infections.
