分段探视模式对改善CCU患者焦虑情绪及睡眠质量的作用

Effect of Controlled Periods for Family Member Accompany on Psychology and Sleep Quality of Patients in Coronary Care Unit

目的探讨三餐分段探视模式对增加冠心病重症监护病房(coronary care unit,CCU)患者睡眠时间、减少焦虑及抑郁情绪的作用。方法将106例入住CCU的老年冠心病患者随机分为观察组(三餐时间分段探视)53例和对照组(下午集中探视)53例,比较两组患者Zung自评抑郁量表(SDS)和焦虑量表(SAS)的评分及睡眠情况。结果两组患者均存在轻度抑郁症状,组间无统计学差异;对照组有轻度焦虑表现,观察组处于正常水平,且两组有统计学差异(P<0.05);观察组患者累计每天睡眠时间为(6.7±1.1)h,对照组为(3.6±2.4)h,两组比较差异有统计学意义(P<0.05)。结论实施三餐时间分段探视,有利于改善CCU患者的焦虑情绪及睡眠情况,减轻护士的工作量;但鉴于可能给患者家属造成的不便,仍需进一步完善CCU患者的分段探视模式。

Objective To assess the feasibility of controlled periods of family member visits for increasing the sleeping time, reducing anxiety and depressive emotion of patients in the coronary care unit（CCU）. Methods A total of 106 elderly patients with coronary artery diseases admitted in CCU were randomly divided into 2 groups. In the observation group, the relatives of the patients were allowed to visit the patients at dinner time, and in the control group, the relatives visited the patients only in the afternoon. The 2 groups were compared for scores of Zung self-rating depression scale （SDS） and anxiety scale （SAS） and for sleeping quality. Results The patients in both groups exhibited mild symptoms of depression, but without statistically Significant difference. The patients in the control group bad mild symptoms of anxiety, whereas those in the observation group had normal anxiety level, showing significant difference between the two groups （ P 〈0.05）. The daily accumulative sleeping time of the patients in the observation group reached 6.7± 1.1 h, significantly longer than that in the control group （3.6 ±2.4 h, p 〈0. 05）. Conclusion Family member visits at the dinner time may improve the anxiety and sleeping quality of the patients in CCU and helps reduce the workload of the nursing staff. But due to the potential inconvenience of this arrangement for the relatives, the arrangement of the visits in CCU should be further optimized.
