摘要
目的探讨PTEN蛋白在喉鳞癌中的表达及意义。方法检索中国学术期刊全文数据库、维普及万方数据库,收集1997~2006年国内公开发表论文,均为PTEN在喉鳞癌中表达及意义的独立病例对照研究。本文对文献进行异质性检验,以病例组及对照组比值比(OR值)为效应指标,然后应用Meta分析软件RevMan4.2.9对各研究原始数据进行统计处理,计算合并OR值及95%可信区间,同时绘制Meta分析森林图和倒漏斗图。结果最终进入系统评价的文献共有9篇病例对照研究,其中喉癌患者487例,对照228例,Meta分析结果合并OR值为0.12,95%可信区间(CI)为[0.07,0.21],PTEN表达与喉癌临床分期、分化程度、淋巴结转移、远处转移及生存率之间存在明显相关(P<0.05);而与喉癌分型、TNM分期、年龄及性别之间无明显相关(P>0.05)。结论对目前相关研究结果的Meta分析显示,PTEN低表达可能在喉癌的发生、发展、转移及预后中起重要作用,这对于喉癌的早期诊断、治疗和预防具有参考意义。
Objective To study the expression and significance of PTEN in laryngeal carcinoma. Methods Studies (published papers in China) from 1 9 9 7 to 2 0 0 6 were retrieved through CNKI, VIP, and WanFang databases. The inclusive criteria of studies must be the case-control ones associated with PTEN and laryngeal carcinoma. To take into account the possibility of heterogeneity of the studies, the authors performed a statistical test for it. Odds ratio and 9 5 % CI were selected to evaluate the risk of the expression of PTEN in laryngeal carcinoma. The Meta-analysis was applied with RevMan 4. 2. 9 software, and the forest plot and funnel plot of Meta-analysis were worked out. Results Nine of the case-control studies were selected for this Meta-analysis with a total of 487 laryngeal carcinoma cases and 228 controls included. The pooled odds ratio was 0. 12, and 95% CI was [-0. 07,0. 21 ]. The expression of PTEN was related to the clinical stage ( P 〈 0. 05 ) , differentiation ( P 〈 0. 05 ) , lymph node metastases ( P 〈 0. 05), distant metastases( P 〈0. 05), and survival rate( P 〈0. 05), but was not related to the tumor site( P 〉0. 05), TNM classification( P 〉0. 05), gender( P 〉0. 05), and age( P 〉 0.05 ). Conclusion The results of the Meta-analysis of the 9 studies reveals the relationship between PTEN and laryngeal carcinoma. PTEN may play an important role in the carcinogenesis, progress, and metastasis of laryngeal carcinoma, which might be valuable for the early diagnosis, treatment, and prognosis of this tumor.
