原发性高血压患者的左房功能变化

The Change of Left Atrial Function in Patients with Essential Hypertension

目的探讨原发性高血压患者左房功能变化与左室重量的关系。方法应用彩色多普勒超声心动图对８６例原发性高血压患者和３５例正常人的左室舒张充盈期各部分血流特征进行分析。结果发现高血压患者的左房最大容积、房缩前容积及左房射血ＰＶＡ、Ａｉ及ＡＥＶ均大于对照组（Ｐ＜０．０５），而房缩前左室充盈显著下降（Ｐ＜０．０５），其中快速充盈容量与对照组无明显差异，仅在管道容积（ＣＶ）部分减少。这些变化在重度心肌增重组更为明显（Ｐ＜０．０５），Ａｉ和ＡＥＶ与ＬＶＭＩ呈正相关（Ｐ＜０．０５）。结论本研究提示高血压患者房缩前左室充盈受损，表现为ＣＶ降低，左房收缩期排血量的增加补充了左室充盈的不足，从而维持心排血量的相对稳定。

Objective To determine the change of left atrial function in patients with essential hypertension.Method\ By using color Doppler echocardiography,left ventricular blood flow parameters were assessed in 86 hypertensive patients and 35 nortensive controls.Results\ It was found that Vmax,PV,and PVA,Ai,AEV in hypertensive patients were higher than those in controls(P<0.05) and LV filling volume before left artium(LA) contraction decreased significantly (P<0.05).Rapid filling volume has no difference compared with that in normal controls,only conduct volume(CV) was decreased.The changes were more remarkable in hypertensive patients whose LVMI increased significantly(P<0.05).A significant relationship between the Ai ,AEV and LVMI was found(P<0.05).Conclusions\ It was showed that LV filing before LA contraction was impaired with the reduction of CV in hypertensive patients.The insufficiency of LV filling was supplemented by the increased LA emptying during atrial contraction,therefore,the cardiac stroke volume was maintained.
