电针配合耳穴治疗单纯性肥胖症的疗效及对抵抗素和血脂水平的影响

Efficacy of Electroacupuncture plus Auricular Plaster Therapy in Treating Simple Obesity and Its Influence on Resistin and Blood Lipid Level

目的观察电针配合耳穴贴压治疗单纯性肥胖症患者的疗效与胰岛素抵抗、血清抵抗素、血脂的变化和相关性。方法对60例单纯性肥胖症患者进行电针配合耳穴治疗。治疗前后测定体重指数(BMI)、胰岛素抵抗指数(HOMA-IR)、抵抗素(RESISTIN)、空腹胰岛素(FINS)、空腹血糖(FBG)、胰岛素敏感指数(IAI);血清总胆固醇(TC)、血清高密度脂蛋白胆固醇(HDL-C)、血清低密度脂蛋白胆固醇(LDL-C)、血清甘油三酯(TG),并与40例正常人对照。结果单纯性肥胖症患者均存在不同程度的胰岛素抵抗及血脂偏高甚至高脂血症的情况,其中RESISTIN、FINS及血脂浓度较正常人明显增高,经过电针配合耳穴治疗后,患者的血脂水平和胰岛素抵抗的情况有了明显改善,BMI、RESISTIN、HOMA-IR有明显降低,IAI升高。结论针刺可以有效改善胰岛素抵抗,并通过一定途径降低食欲,减少了脂肪的摄入,从而降低了血脂的浓度。

Objective To investigate the correlations between the efficacy of electroacupuncture plus auricular plaster therapy in treating simple obesity and changes in insulin resistance, serum resistin and blood lipid. Methods Sixty simple obesity patients were treated with electroacupuncture plus auricular plaster therapy. Zhongwan （ CV 12 ）, Tianshu （ ST 25 ）, Shuifen （ CV 9）, Guilai （ ST 29 ）, Zhongji （ CV 3 ）, Riyue （ GB 24 ）, Qimen （ LR 14 ）, Liangqiu （ ST 34 ） and Yinlingquan （ SP 9 ） were selected for body acupuncture and ear points Shenmen, Waibi, Lung, Spleen, Liver, Large intestine, Small Intestine, Endorine and adrenal gland for auricular plaster therapy. Body mass index （ BMI）, homeostatic model-insulin resistance （ HOMA-IR）, resistin, fast insulin （ FINS）, insulin action index （IAI）, serum total cholesterol （TC）, serum high density lipoprotein-cholesterol （HDL-C）, serum low density lipoprotein-cholesterol （LDL-C） and serum triglyceride （TG） were measured and compared with those in 40 normal persons before and after treatment. Results There were varying degrees of insulin resistance and slightly high blood lipid and even hyperlipidemia in the patients with simple obesity. Of them, resistin, FINS and blood lipid concentration were significantly higher than those in the normal person. Blood lipid level and insulin resistance significantly improved, BMI, resistin and HOMA-IR significantly decreased and IAI increased in the patients after treatment with electroacupuncture plus auricular piaster therapy. Conclusion Acupuncture can effectively reduced insulin resistance and lower blood lipid concentration by reducing appetite through a certain way and decreasing fat intake.
