2型糖尿病微血管病变患者血小板参数及红细胞水平的变化

Clinical significance of platelet parameters and mean corpuscular volume in Type 2 diabetic microangiopathy

目的探讨2型糖尿病微血管病变患者血小板参数及红细胞的变化及其临床意义。方法选择100例2型糖尿病患者分为微血管病变组和无微血管病变组及90例正常对照组为研究对象,采用全自动血细胞分析仪测定血小板计数(PLT)、血小板平均体积(MPV)、血小板平均分布宽度(PDW)、血小板压积(PCT)及红细胞平均体积(MCV)和红细胞分布宽度(RDW)。结果2型糖尿病患者MPV、PDW、MCV、RDW升高,PCT减低,有微血管病变者MPV、PDW、RDW与正常对照组比较升高更加明显(P<0.01,P<0.05)。其中MPV、PDW和FPG与无微血管病变组比较增加显著(P<0.05),MPV与PDW呈正相关(r=0.4,P<0.01),MPV与FPG呈正相关(r=0.77,P<0.01)。结论血小板参数及红细胞测定对2型糖尿病微血管病变的早期诊断和病情分析有临床参考意义。

Objective To explore Clinical significance of of platelet parameters and mean corpuscular volume in Type 2 diabetic microangiopathy disease. Methods The platelet count （PLT）, mean platelet volume （MPV）, platelet pressure accumulates （PCT）, platelet distribution width （ PDW）, mean corpuscular volume （ MCV）, erythrocyte hemoglobin distribution Type 2 width （RDW） and fasting plasma glucose （FPG） were measured in 100 cases with diabetes （50cases with microangiopathy, 50 cases without microangiopthy） and 90 healthy controls. Results The levels of MPV, PDW, MCV and RDW were higher and PCT level was lower in type 2 diabetes patients. Those platelet parameters were significantly high in patients with microangiopath than those in controls （ P 〈 0.01, P 〈 0.05）. The level of MPV, PDW and FPG were higher than those in patients without microangiopath （ P 〈 0.05）. There was positive correlation between the levels of MPV and FPG in diabetes with and without microangiopathy groups （ r = 0.77, P 〈 0.01 ） and between the levels of MPV and PDW （ r = 0.4, P 〈 0.01 ）. Conclusion The measurement of MPV has very important clinical value in the early diagnosis of diabetes with microangiopathy.
