血清TGF-β_1与出血性卒中临床预后的关系

摘要 目的探讨转化生长因子-β1,(TGF-β1)对出血性卒中后脑损伤和临床预后的影响。方法检测103例急性出血性卒中患者血清TGF-β1水平并与健康对照组比较,以3个月Barthel指数评分为临床预后指标对各因素进行单因素与多元分析,观察影响出血性卒中临床预后的因素。结果出血性卒中患者急性期(72h)血清TGF-1β,(51.437±17.330ug/L)低于健康对照组(67.390±16.420μg/L)(P<0.01)。单因素分析TGF-β1(r=0.328,P<0.05),住院天数(r=0.315,P<0.05)使出血性卒中患者神经功能恢复好。NSE高(r=-0.617,P<0.01),意识障碍程度重(r=0.647,P<0.01),出血量大(r=0.627,P<0.01),入院神经功能缺损评分高(r=-0.542,P<0.01),空腹血糖高(r=-0.479,P<0.01),外周血白细胞计数高(r=0.429,P<0.01),发热(r=0.602,P<0.01),最高体温高(r=0.705,P<0.01),患者年龄大(r=-0.379,P<0.05),合并感染(r=-0.368,P<0.05)者神经功能恢复差。多元分析,血清TGF-β1、出血量、意识障碍程度、入院神经功能缺损评分影响出血性卒中患者临床预后。结论TGF-β1,在出血性卒中患者发挥脑保护作用。患者出血量大,意识障碍重,起病时神经功能缺损重,合并感染、发热、白细胞升高、高血糖使临床预后差,神经功能恢复差。
