关节镜下自体镶嵌式骨软骨移植治疗膝关节软骨缺损

Evaluation on clinical effects of autograft cartilage transplantation in arthroscopy in repair of cartilage defect of knees

目的对关节镜下自体镶嵌式骨软骨移植修复软骨缺损的方法及临床疗效进行评价。方法自2001年5月始,在关节镜下行自体镶嵌式骨软骨移植修复软骨缺损11例16膝,术后早期配合使用CPM行患肢被动功能锻炼1周,2～6周行患肢不负重主动功能锻炼。结果术后随访2～4年,平均3年。所有患者膝关节活动良好,疼痛症状基本消失。采用HSS膝关节评分标准综合评价,术后膝关节功能优10膝、良4膝,优良率87.5%。结论关节镜下自体镶嵌式骨软骨移植修复软骨缺损手术损伤小、恢复快,疗效好。

Objective To investigate the clinical effects of autograft cartilage transplantation in acthroscopy in repair of cartilage defect of knees . Methods 16 knees of cartilage defect of knee joints in 11 case treated from May 2001 till now were reviewed . Autograft cartilage was transplanted to repair the defect in arthroscopy during the operation. Early continuous passive movement was adopted for 1 week and active unweight - bearing movement for later 5 weeks postoperatively . Results A following - up was carried out 2 to 4 years postoperatively . The knee joint operated had good motion extend. The symptom of pain was alleviated significantly. According to the HSS system, we found the general satisfactory rate was 87.5% , in which the function was excellent in 10 knees and good in 4 knees . Conclusions Autograft cartilage transplantation in arthroscopy is a good choice in treating cartilage defect because of its satisfactory outcome, with less trauma and quick convalescence .
