血肿清除加白膜修补术治疗阴茎折断

Eliminating hematoma plus repairing white-membrane in the treatment of the penile fracture

目的总结外科治疗阴茎折断的经验。方法报告5年来笔者所在医院收治的8例阴茎折断临床资料。8例均为阴茎海绵体破裂,4例行彩色多普勒超声检查,均发现海绵体破裂口。所有患者均急诊行阴茎血肿清除加白膜修补术治疗。结果1例出现尿瘘,对症处理后痊愈,余7例术后恢复均顺利。8例患者随访6～60个月,均无阴茎弯曲、硬结和成角,无勃起疼痛及勃起功能障碍。结论阴茎折断的诊断主要依据典型病史和临床表现,彩色多普勒超声是诊断阴茎折断的有效辅助检查;急诊手术治疗效果良好,应该列为首选。

Objective To summary the experience of surgical treatment of the penile fracture. Methods The 8 cases of the penile fracture for 5 years recently admitted in author＇s hospital. Injury involved corporeal rupture in all of the 8 cases,the chinks were discovered by Doppler ultrasound in 4 eases. All of the 8 eases received emergency operation eliminating hematoma plus repairing white- membrane. Results Urinary fistula occurred in 1 case, and was healed after symptomatic treatment. The postoperative recovery of other 7 cases was successful. The 8 cases were followed up from 6 months to 5 years after the operation. There were no phallocampsis,sclerosis, angulation, erection ache or erectile disfunction in all the eases. Conclusion Diagnosis of the penile fracture is chiefly based on typical history and clinical manifestation. Doppler ultrasound is the efficient auxiliary examination in the diagnosis of the penile fracture. Emergency operation can give a fine prognosis and should be the first choice.
