放射治疗加微波热疗治疗颈部淋巴结转移癌的近期疗效观察

The late clinical observation for metastatic cervical node treated with Radiation therapy and microwave therapy

目的:探讨放射治疗与微波热疗联合治疗颈部淋巴结转移癌的临床疗效。方法:对58例颈部淋巴结转移癌患者行单纯放疗和放疗加微波热疗,单纯放疗组:采用直线加速器6 MV-X和12 MeV-E/9 MeV-E治疗,累计肿瘤剂量60 Gy～70 Gy/30次～35次/6周～7周;放疗加微波热疗组:放疗同单纯放疗组,微波热疗采用WE-2102A型微波治疗机对颈部淋巴结转移灶区域局部体外加热。30 min/次,2次/周,6次～8次为1个疗程,治疗结束后3个月进行疗效评价。结果:单纯放疗组CR 31.0%(9/29),放疗加微波组CR 58.6%(17/29),两者之间比较差异有显著性(x^2=4.462,P=0.035);总有效率(CR+PR)单纯放疗组65.5%(19/29),放疗加微波组89.7%(26/29),两者之间差异有显著性(x^2=4.858,P=0.028)。结论:颈部肿瘤转移癌单纯放疗效果差,但放射合并加温治疗可以提高疗效。

Objective To detect the clinical efficacy of metastatic cervical node with Radiation therapy and microwave therapy. Methodsfifty-eight metastatic cervical node patients were treated with Radiation therapy alone and Radiation therapy with microwave therapy. Radiation therapy group alone：liner accelerator 6MV-X and 12MeV-E/9 MeV-E were used to give （60- 70） Gy in 6 to 7 weeks; Radiation therapy and microwave therapy group：WE-2102A microwave machine was used to treat metastatic cervical node. 30 minutets every time ,2 times in one week ,6-8 times in one course. We evaluated the clinical efficacy 3 months later. Results Complete responses rates were 31.0% （9/29） and 58.6% （17/29） in radiation therapy group alone and radiation therapy with microwave therapy group, respectively. There was obvious difference of response between the two groupes （χ^2 = 4.462,P = 0.035 ）. Objective confirmed responses were observed in 19 patients （65.5 % ） and 26 patients （89.7%） in two groups, including complete responses and partial responses , There was obvious difference of response between the two groupes （ χ^2 = 4.858, P = 0.028 ）. Conclusion The efficacy of metastatic cervical node with radiation therapy a- lone was poor, but with microwave therapy can improve the efficacy.
