两种动脉重建方式对移植肾远期影响的彩色多普勒超声观察 被引量:1

摘要 目的:通过对5年以上供肾动脉与髂外动脉端侧吻合或与髂内动脉端端吻合的移植肾进行二维超声、多普勒血流参数及血肌酐(SCr)的比较,评价两种动脉重建方式的远期效果。方法:根据供肾动脉的不同吻合方式将64例肾移植患者分为ESA组和EEA组,采用彩色多普勒超声进行监测,观察移植肾的大小、结构、血流灌注情况、血流动力学参数,进行对比分析。结果:两组移植肾患者的SCr、移植肾体积、锥体大小、肾皮质厚度、肾动脉内径、移植肾段动脉、叶间动脉、弓形动脉血流及阻力指数差异均无统计学意义(P>0.05)。结论:供肾动脉与髂外动脉端侧吻合或与髂内动脉端端吻合5年以上的移植肾患者在肾功能、二维超声、多普勒血流上没有明显差异。 Objective To assess the long-term effects of an end-to-side anastomosis (ESA) of renal artery to external iliac artery and an end-to-end anastomosis (EEA) of renal artery to internal iliac artery in renal transplantation by comparing the two-dimensional images, Doppler blood flow parameters, and serum creatinine (SCr) levels in patients after five years of renal transplantation. Methods 64 patients who had undergone kidney transplantation were enrolled in either EEA or ESA group according to the type of anastomosis. Renal blood perfusion, hemodynamic parameters, and renal size and structure were measured in the two groups by color Doppler flow imaging and then compared. Result No statistically significant differences were observed between the two groups in SCr, renal size, centrum size, thickness of the renal cortex, renal artery diameter, and hemodynamic parameters. Conclusion There are no differences between the patients with EEA and those with ESA in renal function, 2D images and color Doppler blood flow parameters.
