蓝光毯与蓝光箱治疗新生儿黄疸的疗效比较

Clinical effect comparison and nursing of neonatal jaundice treatment by BL blanket and neonate photo - therapy warmer

目的比较蓝光毯与蓝光箱对新生儿黄疸的疗效及讨论相应护理问题。方法104例患儿，分两大组，204μmol／L〈血清胆红素〈255μmol／L者为甲组，255μmol／L〈血清胆红素〈342μmol／L为乙组。每组再分为蓝光毯治疗组及蓝光箱治疗组，比较两者之间疗效及副作用。结果甲组中蓝光毯组有效率76、92％，蓝光箱组84、61％，两者差异无显著意义（P〉0．05）。乙组中蓝光毯组有效率50％，蓝光箱组92．30％，两者差异有显著意义（P〈0．01）。蓝光毯组无明显副作用，蓝光箱组则有发热、烦躁、呕吐、腹泻、皮疹等副作用。结论轻中度黄疸以选蓝光毯为宜，重度黄疸则选蓝光箱，但有一定副作用，应采取相应护理措施以达到最佳光疗效果。

Objective To compare the curative effect of neonatal jaundice treated by BL（ blue light） blanket and neonatal photo - therapy Warmer. And to discuss the related nursing problem. Method 104 cases of new born with jaundice were divided into two groups, group A, serum bilirubin 〈255μmoL/L, group B, serum bilirubin 〈342μmol/L, each group divided into two part , treated with BL blanket and neonate photo - therapy warmer respectively. Curative effect and side effect were compared. Results The ratio of curative effect for BL blanket group in group A was 76.92% , and 84.61% in photo - therapy group. There is no significantly difference between them（ P 〉 0.05 ） ,the rate of curative effect of bilirubin group in B group was 50% , and 92.30% for photo - therapy warmer group. There is significantly difference between the two group （P 〈 0.01 ）. There was no apparent side effect in BL blanket group, but there are side effect in neonate photo- therapy warmer group, such as fever, dysphasia, vomiting, diarrhea , rash and so on. Conclusions It is good for low mild jaundice to be treated with BL blanket and for serious jaundice with neonate photo -therapy warmer. There are some side effects and nursing problem with neonate photo -therapy warmer, some nursing methods must be adaped to reach the best photo -therapy effect.
