颈前路一体化钢板椎间融合器对颈椎曲度和椎间高度的影响 被引量:2

摘要 目的探讨颈前路一体化钢板椎间融合器(PCB)对颈椎曲度和椎间高度的影响和临床意义。方法颈椎前路椎间盘摘除术后,选用适当型号的一体化钢板椎间融合器(PCB)置入,无需透视帮助即可拧入螺钉和植骨。治疗脊髓型颈椎病40例,颈椎外伤伴有急性椎间盘突出症18例,外伤性颈椎不稳10例,43例患者接受单节段固定,22例患者为双节段固定,3例患者为三节段固定。本组随访4-36个月,平均17个月。结果椎间隙高度平均增加了32.5%,经过术前和术后的比较,有显著差异(P<0.01);颈椎的前弯角度较术前变化率为125.9%,术后较术前有明显显著差异性(P<0.01);未见颈椎间不融合病例。结论PCB系统在临床应用中不仅有助于恢复颈椎的生理曲度及椎间高度,并且有助于长期维持,提高了椎间融合率和颈椎柱稳定性。 Objective To evaluate the effect of the integrated anterior cervical plate cage benezech(PCB) implant on the overall cervical curvature and middle disc height. Methods After a standard anterior cervical disectomy and preparation of the disc space, the correct size of PCB implant was inserted. Self-tapping screws were inserted without any intensifier. Cancellous bone chips were used, Fourty-three eases underwent fixation by one segment, twenty-two by two segments and three by three segments. The technique was used in 68 cases that were followed up for 4-36 months (average 17 months), of which there 18 with cervical trauma complicated by acute intervertebral disc protrusion and 40 with cervical spondylitic myelopathy, and 10 patients who suffered from fracture dislocation instability of cervical spine without locked joint. Results The postoperative middle disc height was medially increased 32.5% than preoperative one, there was obviously difference between preoperative datas and postoperative ones (P〈0.01). And the postoperative overall cervical curvature was medially increased 25.9% than preoperative one, there was obviously difference between preoperative datas ad postoperative ones (P〈0.01). No non-fusion ease of cervical vertebrae interbody was found. Conclusions The integrated anterior cervical plate cage benezech (PCB) implant conduces not only to the of recovery the overall cervical curvature and middle disc height, but also to the long-term maintenance of ones, that elevates the fusion rate of cervical vertebrae interbody and spinal stabilization.
