宫腔镜电切术后放置T型节育器预防粘连45例临床观察 被引量:10

摘要 目的:宫腔镜电切术后放置T型节育器预防宫腔或宫颈管粘连。方法:行宫腔镜电切术45例,于电切手术结束应用催产素10~20U静脉注射后观察无出血,探测宫腔深度后放置T型节育器。根据电切类型不同,于术后1~3个月取出节育器,并行宫腔镜检查,判断是否存在宫腔或宫颈管粘连。结果:宫腔镜电切术45例。术后3个月随访并行宫腔镜检查,除宫腔粘连分离术4例中粘连1例(25.00%);其余均未发现宫腔粘连。结论:宫腔电切术放置T型节育器对预防宫腔或宫颈管粘连有效,有效率达97.78%。利用T型节育器的两侧横臂及带尾丝的纵杆将子宫颈管及宫腔前后壁分开,预防粘连,且取器容易、方便,值得推广。 Objective: To explore the prevention of adhesion of uterine cavity or endocervical canal by installation of T type intrauterine device after hysteroscopic electroresection. Methods: Forty people intending to install intrauterine device underwent hysteroscopic electroresection, then infused oxytocin 10 ~20 U intravenously , detected depth of uterine cavity after bleeding had ceased and installed T type intrauterine devices. The intrauterine devices were withdrawn in 1~3 months according to different electroresection; then underwent hysteroscopy to investigate whether there was adhesion of uterine cavity or endocervical canal. Results: Forty five cases were follow - up and underwent hysteroscopy in 3 months after electroresection, one of the 4 cases who had undergone separation for the adhesion of uterine cavity took place adhesion(25.00% ), the others without adhesion. Conclusion: T type intrauterine device installation after hysteroscopic electroresection can prevent adhesion of uterine cavity or endocervical canal effectively, the rate of efficiency was 97.78%. With transverse bars on both sides and longitudinal rod with thread tail , the T type intrauterine device is able to prevent adhesion by separating anterior wall and posterior wall of the uterine cavity as well as endocervical canal, can be withdrawn easily. It deserves generalization.
