乌拉地尔对心力衰竭患者的血液动力学效应

Acute hemodynamic effects of intravenous urapidil in heart failure

乌拉地尔是α１受体阻滞剂，可用于心力衰竭治疗。本组观察了１１例缺血性心脏病心力衰竭患者静脉滴注乌拉地尔２～１２μｇｋｇ－１／ｍｉｎ的血液动力学效应。结果表明：乌拉地尔可降低总外周阻力、肺毛细血管楔压、肺血管阻力、肺动脉压，增加心排血量、心脏指数、左室作功指数，而心率、平均动脉压与用药前比较无明显统计学差异。因此，结果提示，静脉滴注乌拉地尔对缺血性心脏病所致心力衰竭有良好的血流动力学效应。

Urapidil, an alpha adrenoceptor blocking drug, has been shown to be effective in the treatment of hypertension and congestive heart failure. The purpose of the study was to evaluate the acute hemodynamic effects of urapidil. Eleven patients with heart failure due to ischemic heart disease were included in the study. Seven of them were acute left heart failure and the other were chronic congestive heart failure. Urapidil was given by infusion at a rate of 212μg·kg -1 /min, starting at 2 or 4 μg·kg -1 /min. In all patients, the hemodynamic record was performed 10 minutes and 30 minutes before and after urapidil infusion. Systemic vascular resistance, pulmonary vascular resistance, pulmonary artery wedge pressure, mean pulmonary pressure were decreased; cardiac output, cardiac index and left ventricular stroke work index were increased after urapidil infusion. There were no significant alterations in heart rate, mean arterial pressure, and right atrial pressure. No serious side effects were observed in the study. The study demonstrated that intravenous urapidil produced acute hemodynamic benefit in patients with acute and congestive heart failure.
