体外游离肝灌注系统模拟兔-人异种肝移植

摘要 目的:选用家兔作为器官移植供体,以灌流泵、氧合器、恒温系统及监测系统建立体外器官灌注模型模拟兔-人异种肝移植。方法:将家兔自身血(n=4)、健康人全血(n=4)及人血浆(n=4)分别引入此灌注系统。结果:自身对照及人血浆灌注组持续灌注120min门静脉灌注压均为正常水平。人血灌注组于(15.0±1.2)min出现超急排斥反应。ELISA天然抗体IgM测定结果则表明,人全血灌注家兔肝脏后IgM吸附率为(39.4±5.5)%,血浆吸附率为(35.3±3.5)%,对照组IgM吸附率为(9.2±1.0)%。免疫荧光病理发现人全血组可见明显IgM荧光抗体沉积在肝毛细血管并见毛细血管微血栓形成,血浆组也可见IgM沉积,对照组则无沉积。结论:应用体外游离肝灌注系统可以使模拟兔-人异种肝移植成为可能,模型稳定,重复性强,为实验性异种移植提供有意义的资料。 Objective: To build an isolated perfusion system to imitate xenograft between rabbit to human.Methods: Rabbit liver grafts were placed in an ex-vivo circuit perfused with: (a) autologous blood (n = 4),(b) human boold (n = 4 ) and (c) human plasma. Results: The pressure of PV remained normal during perfusion of rabbit liver with rabbit blood (autologous ) for up to 120 min. In group b (human blood) rejected promptly within (15 ± 1. 2) min. The result of ELISA showed that group b (human blood ) exhibited a dramatic reduction of natural antibody (IgM) activity [absorption rate being (39. 4±5. 5 ) % ]. In human plasma group little change [ (35. 3 ± 3. 5) % ] in IgM was detected as compared with that of the group control [ (9. 2 ± 1.0) % ]. The immunopathological analysis of tissue and antibody assessment was always consistent with hyperacute rejection (group b). Conclusion: Isolated perfusion systern has been a reproducible, reliable system.
