摘要
目的:本课题从寰枢关节错位是枢椎齿状突偏移或倾斜主要是上段颈椎椎曲改变的生物力学原理立论。应用调曲端转复位法治疗寰枢关节错位,并观察其疗效及安全性。方法:通过采用双盲随机方法,分为用端转法治疗为主的对照组,和运用调曲法端转法为主治疗的治疗组。观察了符合纳入标准病人96例,其中对照组38例,治疗组58例。二组病人均采用针刺方法,但复位手法分别是对照组用端转法,治疗组用调曲端转法,连续治疗两周,每7天1疗程,中间休息1-2天,再治疗7天,共14天,结束疗程。
结果:经统计学检验,对照组总有效率为84.2%,临床治愈率为21.1%;治疗组总有效为98.9%,临床治愈率为93.1%。治疗组疗效高于对照组,差别具极显著性意义,P〈0.05。治疗观察期间无不良反应,并通过颈椎和寰枢关节生物力学分析,说明颈调曲端转法是治疗寰枢关节错位安全、可靠、高疗效的方法。
Methods: This thesis was to researched the biomechanics of the odontoid process deviation or tilting(the main was the upper cervical curvature changed) was the malposition of atlantoaxial joint. To used way of curvature adjusting lifting and rotating method to treat malpositon of atlantoaxial joint and observed its effective and safety. Ways: We used Randomized doubled blinded to separated the control group which was used the way of lifting and rotating method to treat the malposition of atlantoaxial joint and the operated group which was used the curvature adjusting lifting and rotating method. Observed 96 patients who was suit for the Standard and the control group had 38 patients, the operated group had 58 patients. These two groups all used the way of pin-prick, but the manual way was different. In control group we used lifting and rotating method, in operated group we used curvature adjusting lifting and rotating method. The courses of treatment were 7days, and have one or two days rest, then 7days continuous. All the courses were taken 14days. Results: Though the Statistics, the total effects rate of control group was 84.2%, the clinical curative effects was 21.1%; and the total effects rate of operated group was 98.9%; the clinical curvature effects was 93.1%, the operated group effects was better than the control group. P〈0.05, the difference was remarkable. And there was no reaction. Through the cervical vertebrae and the atlantoaxial joint biomechanics analysis, showed that the curvature adjusting lifting and rotating method was safety for malposition of atlantoaxial joint treatment, and it can get excellent effect.
