摘要
目的:检测人周围型肺癌组织中MMP-2、VEGF蛋白的表达,并探讨肺癌生长、浸润和转移的影像学表现及其分子生物学特性。方法:采用免疫组织化学方法检测57例周围型肺癌(鳞状细胞癌19例,腺癌34例,肺泡癌4例;蛋白Ⅰ级9例,Ⅱ级24例,Ⅲ级24例)组织中MMP-2、VEGF蛋白的表达,并分析其与螺旋CT影像学征象的关系。结果:肺癌组织中MMP-2、VEGF蛋白的表达与组织学类型无关(P均>0.05),与病理学分级相关(P均<0.05)。肺癌组织中MMP-2蛋白表达与螺旋CT征象中肿瘤直径、深分叶征、棘突征、纵隔淋巴结转移有关(P均<0.05),与毛刺征、蛋白胸膜凹陷征无关(P均>0.05)。肺癌组织中VEGF蛋白表达与螺旋CT征象中肿瘤直径、深分叶征、棘突征、胸膜凹陷征、纵隔淋巴结转移有关(P均<0.05),与毛刺征无关(P>0.05)。且周围型肺癌的强化程度与MMP-2、VEGF蛋白表达有关(P均<0.05)。结论:螺旋CT是诊断周围型肺癌可靠的检查方法。MMP-2、VEGF蛋白的异常表达与周围型肺癌的病理学分级、螺旋CT征象相关,可以在蛋白表达基础上判断周围型肺癌的侵袭、转移和预后。
Aim: To detect MMP-2 and VEGF in human peripheral lung carcinoma tissue,and investigate its imaging performance and molecular biological character of the growth, invasion and metastasis of lung carcinoma. Mothods:A total of 57 cases of peripheral lung carcinoma specimens( 19 cases of squamous cell carcinoma, 34 cases of adenocarcinoma, 4 cases of bronchioloalveloar carcinoma;9 cases of grade Ⅰ,24 cases of grae Ⅱ ,24 cases of grade Ⅲ ) were collected. MMP- 2 and VEGF expression were detected by using immunohistochemical method. Results:MMP-2,VEGF expression in human peripheral lung carcinoma tissue were related with pathological grade ( P 〈 0.05 ) , were not related with histological type (P 〉 0.05 ). MMP-2 expression was related with features of spiral CT ( P 〈 0.05 ) except spicule sign, pleura umbilieate sign (P 〉 0.05 ). VEGF expression was related with features of spiral CT ( P 〈 0.05 ) except spicule sign ( P 〉 0.05 ). And MMP-2, VEGF expression were related with enhanced level( P 〈 0.05 ). Conclusion:Spiral CT is a reliable method for the diagnosis of peripheral lung carcinoma. The abnormal expression of MMP-2 and VEGF in the peripheral lung cancer tissue are related to pathological grade and CT findings, which makes it possible to evaluate invasion, metastasis and prognosis of the peripheral lung carcinoma in the level of protein expression.
