播散性低分化腺癌并发印戒细胞癌皮肤转移

General cutaneous metastasis with signet-ring cell carcinoma spreading from poorly differentiated adenocarcinoma

报告1例播散性低分化腺癌并发印戒细胞癌皮肤转移。患者男,23岁。躯干、头面部、四肢出现黄豆至鸽卵大结节2个半月,无自觉症状。皮损组织病理检查:真皮胶原纤维间及皮下组织内肿瘤细胞弥漫性浸润,真皮胶原纤维被破坏,肿瘤细胞体积较大,胞质略空,并见瘤巨细胞。免疫组化染色示LCA、CK7、CD138均(-),CK20(+)。诊断:低分化腺癌并发印戒细胞癌皮肤转移。患者于皮肤转移癌出现后8个月死亡。

One case of general cutaneous metastasis with signet-ring cell carcinoma spreading from poorly diffferentiated adenoeareinoma is reported. A 23-year-old man complained of small muhi-masses, soybean or pigeon egg in size, appearing on his taee. extremities and trunk for 2.5 months without any uneomlbrtable feelings. Pathological examination showed ttanor cells were diffuse infiltrated in dermal collagen and subculaneous tissue. Tumor cells were large. Cytoplasm seemed a little empty and giant tumor eells could be seen. The dermal collagens were destroyed. Immuno-histologieal examination showed thai LCA,CK7 and CD138 were negative and CK20 were positive. The poorly differentiated adenoeareinoma with skin metastasis of signet-ring cell carcinoma was diagnosed. The palient was died in 8 months with appearing tumor melaslasis in skin.
