摘要
目的:研究在犬肝移植模型中,肝动脉缺血对血清中TGF-β1、TNF-α、IL-1和IL-6水平的影响及其肝动脉桥式置管转流装置对其的干预作用。方法:24只杂交犬被随机分为正常对照组、肝动脉门静脉同时开放组、肝动脉缺血组和肝动脉桥式置管转流组,每组6只。正常对照组开腹后仅游离肝脏并解剖相应的血管,在此之后0、2、6、12和24h分别抽血留样;肝动脉门静脉同时开放组在冷灌注后,同时开放肝动脉和门静脉;肝动脉缺血组在冷灌注后,先开放门静脉,1h后再开放肝动脉;肝动脉桥式置管转流组在腹主动脉与胃十二指肠动脉之间建立桥式转流通道,在冷灌注后同时开放门静脉和桥式转流通道,1h后再开放肝动脉,夹闭桥式转流通道;后3组均在冷灌注后0、2、6、12和24h分别抽血留样。ELISA法测定血清中的TGF-β1、TNF-α、IL-1和IL-6值。结果:随着时间的延长,血清中细胞因子的值均有所升高,并于6h时达到峰值;与正常对照组比较,其它3组在冷灌注后各时点细胞因子的值均高于正常对照组(P<0.05),肝动脉桥式置管转流组细胞因子的值在冷灌注后各时点的值介于肝动脉缺血组和肝动脉门静脉同时开放组之间,3组之间差异显著(P<0.05)。结论:在犬肝移植模型中,肝动脉缺血可引起血清中TGF-β1、TNF-α、IL-1和IL-6值显著升高;肝动脉桥式置管转流可以有效的抑制肝动脉缺血的这一作用。
Objective: To study the influence of hepatic artery ischemia on the levels of serum TGF - β1, TNF-α, IL- 1 and IL - 6 and the intervention study in liver transplantation model of dog. Methods: 24 dogs were randomly divided into normal control group, hepatic artery ischemia and portal vein open simultaneously group(HPOS group), hepatic artery isehemia group(HAI group) and transferring the blood by a bridging duct group(TBB group) and each group had 6 dogs. In the normal control group, we only dissociated the liver and dissected the blood vessels, the blood sample were collected in 0, 2, 6, 12 and 24 h, respectively, after the operation in HPOS group, the hepatic artery and portal vein were opened simultaneously after the models were established; in HAl group, the hepatic artery was opened 1 h alter the portal vein was opened; in TBB group, the hepatic artery was revascularized 1 h after the portal vein and the bridging duct were opened and then the bridging duct was clamped. The blood samples were collected in 0, 2, 6, 12 and 24 h, respectively, after cold perfusion in these three groups. The levels of serum TGF -β1, TNF - α, IL- 1 and IL - 6 were determined by ELISA. Results:With the time prolonging, the levels of serum cytokines increased and reached to the peak in 6 h. Compared with the normal control group, the levels of all serum cytokines of other three groups were higher(P〈0. 05). The levels of serum cytokines of TBB group in every time after cold perfusion were higher than those of HPOS group (P〈0. 05) and were lower than those of HAI group(P〈0. 05). Conclusion: Hepatic artery ischemia can increase significantly the levels of serum TGF - β1, TNF - α, IL- 1 and IL- 6 in liver transplantation model of dog and the bridging duct which transfer the blood to hepatic artery can inhibit this effect.
