摘要 【目的】评价细胞角质素20(CK20)和存活素(Survivin)在膀胱癌诊断中的价值。【方法】采用前瞻性临床研究的方法,应用RT-PCR技术,对膀胱癌病人、泌尿系良性疾病病人和健康志愿者进行尿脱落细胞CK20mRNA、Survivin mRNA检测和尿细胞学(UC)检查,比较其在膀胱癌诊断中的价值。【结果】尿脱落细胞CK20mRNA、Survivin mRNA检测和UC检查的敏感性分别为86.1%、83.3%、31.9%,特异性分别为63.4%、90.2%、100%。【结论】尿脱落细胞CK20mRNA、Survivin mRNA检测的敏感性均明显高于尿细胞学检查,可以替代尿细胞学检查,作为膀胱镜检查的辅助手段。Survivin mRNA检测具有较高的特异性,适用于膀胱癌的临床诊断。 [Objective]To evaluate cytokeratin 20 (CK20) and survivin as diagnostic markers for bladder tumor. [Methods] The diagnostic values of the expression of CK20 and survivin in urine exfoliate cell taken from patients with bladder tumor and from control groups by means of RT-PCR method were prospectively analyzed. [Results]The overall sensitivities and specificities for CK20, survivin and urinary cytology (UC) were 86.1%, 83.3%, 31.9% and 63.4% , 90.2% ,100%, respectively. [Conclusion] The sensitivity of detection of CK20 mRNA and surviving mRNA in urine exfoliate cell is higher than that of cytology and can be instead of it. The detection of CK20 mRNA and surviving mRNA can be as adjuvant means of cystoscopy. Survivin mRNA has good specific, and is fit for clinical diagnosis of bladder cancer.
