免气腹针Trocar直入法造气腹腹腔镜手术220例应用体会

Experience of 220 cases of laparoscopic surgery by putting trocar to finish pneumoperitoneal without veress needle

目的探讨免气腹针Trocar直入法造气腹腹腔镜手术的临床应用价值。方法对2003年2月～2006年8月的220例妇科腹腔镜手术,采用免气腹针Trocar直入法造气腹作为研究组,随机选同期200例腹腔镜手术,使用气腹针穿刺造气腹作为对照组。结果该组220例均成功穿刺并制造气腹,无1例发生脏器穿孔、腹膜后大血管损伤等严重并发症。结论免气腹针Trocar直入法在腹腔镜手术中应用具有可行性和安全性,值得推广。

[Objective] To investigate the value of laparoscopic surgery not using veress needle but trocar been put in first to finish pneumoperitoneal. [Methods] 220 laparoscopic surgery cases in gynecology were investigated, in which veress needle were not being used but trocar been put in first to finish pneumoperitoneal from Feb 2003 to Aug 2006, and the results were compared with 200 laparoscopic surgery cases using veress needle. [Results] All cases were finished successfully. There were no gastrointestinal and blood vessels damage if it was perfectly operated. [ Conclusion] The method of laparoscopic surgery without veress needle but trocar been put in first to finish pneumoperitoneal is safe and feasible, and worth popularization.
