摘要 目的:探讨虚拟现实技术在颅底肿瘤诊断和手术计划中的应用价值。方法:通过虚拟影像手术计划系统对15例颅底肿瘤患者的影像检查资料进行三维重建,获得颅骨、脑、血管等的三维立体虚拟图像,对相应解剖结构进行多角度观察和测量,运用系统工具进行多入路模拟手术操作。结果:15例患者均成功进行了手术计划和模拟手术操作。该系统形成的三维立体虚拟影像清晰、逼真,可非常直观地显示肿瘤的位置、大小及其与血管、神经、颅骨的解剖关系。全部病例按照最佳模拟手术方案进行手术,均取得了良好的手术效果。结论:虚拟影像手术计划系统的使用,能使术者获得更多的立体的解剖信息,借助模拟手术工具可选择颅底肿瘤的最佳手术入路。 Objective: To investigate the values of virtual reality techniques in diagnosis and operative planning of base skull tumors. Methods: Image data of 15 base skull tumor cases were reconstructed and transferred into 3D mode through the surgical plan system based on virtual reality image, and the 3D solid virtual images of skulls, brain tissues and cerebral vessels were acquired, and their anatomic relationships with adjacent tissues were observed and measured from many points of view, and the multi-approach simulation operations were performed using tools of the surgical plan system. Results: Surgical plan and simulation operation of 15 cases were done successfully. The 3D solid virtual images produced by the virtual imaging system were clear and vivid, which could reveal directly the location and size of the tumor and its relations to the vessels, nerves and skull. The real operation of all patients complied with the best simulated operative plan through the system, and the outcome of these operations were good. Conclusions: Operator can get more 3D anatomic information by using the surgical plan system based on virtual reality image, and the best operative approaches of base skull rumors can be selected by simulating operation tools of the system.
