空蝶鞍综合征鞍区水平脑脊液流动磁共振定量研究 被引量:8

MR quantitative study of sella region of empty sella syndrome cerebrospinal fluid flow

摘要 目的应用磁共振相位对比法(MRI PC-cine)观察空蝶鞍综合征鞍区脑脊液的流动情况,并进行定量分析。方法筛选11例空蝶鞍综合征患者,测量其鞍区脑脊液流动情况,在正中矢状位T1WI或T2WI图像上选择鞍区层面,编码速率20cm/s,计算鞍区水平脑脊液流动方向、流速、流量率等情况并研究其与心动周期的关系。结果空蝶鞍综合征鞍区脑脊液流动在1个心动周期内表现为双向流动,收缩期向下最大流速、舒张期向上最大流速、平均向下流速、平均向上流速、平均流速分别为(1·50±0·91)cm/s[(382·36±281·94)ms]、(1·84±1·36)cm/s[(286·55±276·62)ms]、(0·64±0·67)cm/s、(0·91±0·57)cm/s、(0·69±0·64)cm/s。收缩期向下最大流量率、舒张期向上最大流量率、平均向下流量率、平均向上流量率、平均流量率、平均净流量率分别为(0·015±0·082)ml/s、(0·018±0·013)ml/s、(0·067±0·068)ml/s、(0·077±0·061)ml/s、(0·007±0·055)ml/s、(0·055±0·046)ml/s。平均心动周期为(800·47±141·38)ms。结论MRIPC-cine能无损伤地研究人体脑脊液的流动和比较精确地确定脑脊液的流速、流量率和方向,是一种优于其他方法研究脑脊液流动的重要方法,具有较高的临床应用价值。 Objective To quantitatively study the features of cerebrospinal fluid flow (CSF) dynamics in empty sella syndrome with MRI. Methods Eleven patients of empty sella syndrome were studied. The sella region CSF flow image was positioned sella region at the middle sagittal T1WI or T2WI image. This pulse sequence used a velocity encoding of 20 cm/s. The waveforms were analyzed for the maximum flow velocity, flow volumerate and the change of the figures. From the velocity and area measurements on the cine images, mean CSF flow were calculated in millimeters per second and milliliters per cardiac cycle. Results The patients of empty sella CSF flow of the sella region has two directions. The out and in flow peak velocity and mean out and in flow and mean flow were (1.50± 0. 91 )cm/s [ (382. 36 ± 281.94) ms ] , ( 1.84 ± 1.36 ) cm/s [ (286. 55 ± 276. 62) ms] , (0. 64 ±0. 67 ) cm/s, (0. 91 ± 0. 57 ) cm/s and (0. 69 ± 0. 64 )cm/s respectively. The out and in volumerate peak velocity and mean out and in volumerate and mean volumerate (0. 015 ± 0. 082 ) mL/s, (0. 018 ±0. 013 ) mL/s, ( 0. 067± 0. 068 ) mL/s, ( 0. 077± 0. 061 ) mL/s, ( 0. 007 ±0. 055 ) mL/s and ( 0. 055± 0. 046 ) mL/s respectively. The mean cycle was ( 800. 47± 141.38)ms. Conclusions MRI cine PC is a noninvasive method to study the CSF flow in physiological and pathological conditions for determining the pattern, direction, speed and quantity of the CSF flow. Therefore MRI PC-cine is better than other invasive research modalities ,which has a important value in clinic application.
