胺碘酮治疗急性心肌梗死并心房颤动的疗效

Effect of amiodarone for atrial fibrillation in patients with acute myocardial infarction

目的:观察胺碘酮治疗急性心肌梗死(AMI)并心房颤动(房颤)的临床疗效及安全性。方法:对34例AMI并房颤患者首剂静脉注射胺碘酮150mg,随后以0.5～1.5mg/min维持静脉滴注。静脉用药2～7(3.24士1.7)d。结果:房颤转复率2h内为47.1%(16/34),24h内为70.6%(24/34),48h内为85.3%(29/34),72h内为91.2%(31/34)。治疗期间,5例(14.7%)出现窦性心动过缓,减量后心率恢复4例(11.8%);4例(11.8%)出现血压降低,用多巴胺后血压上升至正常。结论:静脉注射胺碘酮对AMI并房颤疗效明显,未见严重不良反应。

Objective：To evaluate the efficacy and safety of intravenous amiodarone for atrial fibrillation with acute myocardial infarction. Method：Thirty-four patients with acute myocardial infarction and atrial fibrillation were treated with amiodarone. The first bolus of amiodarone was given at 150 mg intravenous followed by 0.5-1 mg/ min in fusion. The effect was defined as returning to sinus rhythm in 24 hour. Result： Sixteen patients were reverted to sinus rhythm （47.1%） within 2 hour, 24 patients were reverted to sinus rhythm（70.6%） within 24. hour, 29 patients were reverted to sinus rhythm（85.3%）, within 48 hour. side effects were observed in 5 patients （14.7 % ）, Conclusion：Amiodarone is effective and safe for treatment of atrial fibrillation with acute myocardial infarction.
