心房颤动山羊模型转复窦性心律后血液和心房肌组织中醛固酮水平的变化 被引量:1

The changes of aldosterone level in blood and atrial tissue after atrial fibrillation revert to sinus rhythm in the goat

摘要 目的:研究心房颤动(Af)山羊模型恢复窦性心律后血液和心房肌组织中醛固酮水平的变化。方法:18只成年山羊,体重相仿,雌雄不限,随机分成3组,其中窦性心律组(A组)6只,Af模型组(B组)6只,Af模型复律组(C组)6只,3组均行开胸手术,左心耳缝入起搏电极,A组不起搏,B、C组持续快速心房起搏3个月形成Af模型,3个月后处死A、B组山羊,取左、右心房肌组织冻存,C组停止起搏,予心律平静脉注射或加用直流电复律,维持窦性心律3个月,之后处死C组山羊,取左、右心房肌组织冻存,用ELISA法测定各组心肌标本中醛固酮水平。C组在Af前、Af后3个月、复律后2周、1个月、2个月、3个月时各留取血清标本,用放射免疫法测定血清醛固酮水平。结果:C组心肌组织醛固酮水平低于B组(P<0.01),与A组比较则差异无统计学意义(P>0.05),左、右心房肌组织醛固酮水平差异无统计学意义(P>0.05);Af复律后2周、1个月、2个月、3个月血清醛固酮水平呈递减变化。结论:Af恢复窦性心律后血液和心房肌组织中醛固酮水平下降。 Objective:To investigate the changes of aldosterone level in blood and atrial tissue when atrial fibril- lation (Af) has been converted. Method:Eighteen goats were randomly divided into 3 groups (each n=6) : ① sinus rhythm (group A) ; ②3 months' Af (group B) ; ③3 months' sinus rhythm after 3 months' Af (group C). Three groups of goats all underwent open chest surgery to implant the pacemaker electrodes into left auricles. Group B and C were given continuous and rapid pacing in order to create Af model, while group A was not. Three months later, group A and B were all killed with their left and right atrial myocardium reserved. Group C was stopped pacing and converted into sinus rhythm by propafenone and/or direct current. After three months' sinus rhythm, group C was killed with atrial myocardium reserved as well. Serum samples were respectively taken when group C was at different periods: pre-pacing, 3 months post-pacing, 2 weeks, 1 month, 2 months, 3 months after Af reversion to sinus rhythm. Atrial aldosterone level was studied by enzyme-linked immunosorbent assay (ELISA). Serum aldosterone level was determined by radioimmunoassay. Result: The aldosterone in atrial tissue decreased significantly in cardioversion group compared with that in Af group (P〈0.01), while it showed no marked differ- ences from that in sinus rhythm group (P〉0.05). The atrial aldosterone was found to be no difference between left atria and right atria in which even group (P=0. 149). The serum aldosterone level decreased gradually when Af had been stopped. Conclusion:The aldosterone level in blood and atrial tissue decreased after Af reversion to sinus rhythm.
