结直肠癌患者血液循环中癌细胞微转移的观察 被引量:4

摘要 目的观察结直肠癌患者手术时癌细胞微转移情况。方法以K19 mRNA及癌胚抗原(CEA) mRNA为标志物,取门静脉血与周围血做反转录-聚合酶链反应(RT-PCR)检测。结果K19 mRNA以及CEA mRNA两者的阳性率大致相符,而且与临床分期、病理细胞分化情况相关(P<0.05)。经过2年的随访,阳性、可疑及阴性组三者之间差异有统计学意义(P<0.05)。结论对结直肠癌患者血液循环中K19以及CEA两种标志物的检测,具有提示临床情况的意义,并且与患者预后有关。可用于对术后患者的定时复查,以使医师及早做出进一步的治疗计划或及时调整现有治疗计划。 Objective To investigate the micrometastases in bloodstream of patients with colorectal cancer at the time of surgery. Methods Blood samples collected from portal and peripheral veins were measured for K19-mRNA and CEA- mRNA with RT-PCR. Results The rates of positive results were similar between these two markers and were well correlated with clinical staging and cell differentiation(P〈0.05 and P〈0.025). After follow-up for two years, the rates of survival were significantly different among those with positive, negative or suspected findings (P〈0.05). Conclusion Determination of K19 and CEA may have clinical implications associated with prognosis. Regular monitoring of these two markers after surgery would allow early therapeutic decision and timely modulation of current treatment.
