冠状动脉心肌桥的临床诊断与治疗

Clinical diagnosis and treatment of coronary myocardial bridge

目的探讨心肌桥的影像特征及临床治疗方法。方法对3055例冠状动脉(冠脉)造影中检出的46例心肌桥患者的临床资料进行回顾性分析。结果心肌桥发生率为1.51%,均为单桥,均位于前降支;临床诊断孤立性心肌桥16例,并发冠脉粥样硬化18例,并发高血压病22例。46例心肌桥患者均有发作性胸痛症状。结论选择性冠脉造影可用于心肌桥的诊断,心肌桥可能引起心肌缺血表现,应用β-受体阻滞剂等药物治疗心肌桥患者有效。

[Objective] To explore the angiographic features of coronary myocardial bridge （CMB） and evaluate its clinical treatment. [Method] 46 patients with CMB out of 3055 cases undergoing selective coronary angiography were studied retrospectively. [Result] The incidence of CMB was 1.51%. All the patients had a single CMB and all these CMB involved the left anterior descending coronary artery （LAD）. 16 patients were diagnosed as simple CMB, 18 patients were accompanied with coronary atherosclerosis, 22 patients with hypertension. All the CMB patients had angina pectoris. Medicine such as β-blocker was effective to treat CMB patients. [Conclusion] Selective coronany angiography can be used to diagnose CMB. CMB may cause myocardial ischemia. CMB patients with symptoms should be treated actively.
