入院SIRS评分预测多发伤患者预后的临床分析

CLINICAL ANALYSIS ON PREDICTIVE VALUE OF SIRS SCORING AT ADMISSION FOR PROGNOSIS OF PATIENTS WITH MULTIPLE TRAUMA

目的探讨多发伤患者入院SIRS评分与其预后的相关性。方法对我院2003年1月—2006年12月收治的883例多发伤患者进行入院SIRS和ISS评分,分析多发伤病死率与SIRS评分的关系,评价SIRS评分与多发伤患者病死率、入ICU率、休克率、住院时间长短等预后的相关性。结果多发伤的休克率、入住ICU率、平均住院天数和病死率均随着SIRS分值的增加而增高的趋势(P<0.01);多发伤后SIRS发生率为37.15%,SIRS组患者与非SIRS组比较病死率明显增加。结论入院SIRS评分操作简单,有独立预测多发伤患者预后的价值。

Objective To explore the correlation between systemic inflammatory reaction syndrome （SIRS） score at admission and the prognosis of patients with multiple trauma. Methods SIRS and ISS scoring of 883 cases with multiple trauma admitted to our hospital were performed during January 2003 to December 2006. Analysis on the relationship between SIRS scoring and the mortality of multiple trauma was carried out, association between the mortality of multiple trauma, rate of entering the ICU, rate of shock, the length of hospitalization the outcome, and the SIRS scoring were evaluated. Results The rate of shock, rate of entering the ICU, average length of hospitalization and mortality increased with the increment of the SIRS score（P〈0.01） ; the rate of SIRS was 37.15 %, the mortality of patients in the SIRS group was significant higher than those in the non- SIRS group. Conclusion SIRS scoring at admission is easy to perform, it has independent predictive value for patients with multiple trauma.
