七氟烷复合瑞芬太尼麻醉用于腹腔镜肾囊肿去顶减压术效果观察

Effects of reminfentanil combined with sevoflurance on retroperitonea laparoscopic for renal cyst

目的观察七氟烷复合瑞芬太尼全麻用于经腹膜后途径行腹腔镜肾囊肿去顶减压术的效果。方法7例腹腔镜下肾囊肿去顶减压术患者,以咪哒唑仑0.05 mg·L^-1、丙泊酚1.5 mg·kg^-1、瑞芬太尼1μg^-1或罗库溴铵1 mg·kg^-1诱导插管,以2%～4%七氟烷吸入、瑞芬太尼10～20μg·kg^-1·h^-1。手术结束前约5 min停止七氟烷吸入,瑞芬太尼于手术结束时停止输入。结果术中ECG、BP、HR相对稳定,气道压〈30 cm H2O,SpO298%～100%,术毕5～10 min内完全清醒拔管。术后随访无并发症及术中知晓。结论七氟烷（2%～4%）复合瑞芬太尼：（10～20μg·kg^-1·h^-1）麻醉用于腹腔镜肾囊肿去顶减压术安全有效、可控性佳。

Aim To evaluate the effects of reminfentanil combined with sevoflurance on patients receiving retroperitonea laparoscopic for renal cyst. Methods Seven adults ASA Ⅰ-Ⅱ patients undergoing retroperitonea laparoscopic for renal cyst were continuously given reminfentanil（10～20μg·kg^-1·h^-1） by a microinfusion pump and 2% ～% sevoflurance by inhalation during operation. NIBP, EKG, HR, SpO2 ,PET CO2, cerebral state index（ CSI ）, and recovery time, the side effects were recorded. Results During operation, hemodynamic and respiratory variables of all patients remained stable. All patients awake 10 min after operation. Conclusion General anesthesia by reminfentanil combined with sevoflurance on retroperitonea laparoscopic for renal cyst is safe and feasible.
