摘要
学术背景:不少研究表明血管内皮生长因子C与恶性肿瘤的淋巴道转移密切相关。血管内皮生长因子C可选择性地诱导淋巴管的增生,在调节血管生成方面与血管内皮生长因子有相同的作用。目的:概述血管内皮生长因子C的结构、分布、受体、功能以及在肿瘤淋巴道转移中的可能作用途径,探索肿瘤细胞的生长及转移因素。检索策略:由作者应用计算机检索Medline数据库、PubMed数据库1995-01/2004-12有关血管内皮生长因子C的文献,限定文章语言种类为英文;另外检索中国生物医学文献数据库及中国学术期刊全文数据库1995-01/2004-12有关血管内皮生长因子C的文献,限定文章语言种类为中文。并辅以文献追溯、手工检索等方法共收集到公开发表的文献405篇。文献纳入标准:①研究对象为人类器官肿瘤,样本量≥30。②对于相似内容文献选择最新的文献。排除标准:①动物研究。②重复报告、质量较差的文献。文献评价:共32篇满足纳入标准,文献来源于综述、个例报道及专题论著。资料综合:①血管内皮生长因子C及其受体血管内皮生长因子受体3是目前发现的惟一一组调节胚胎组织淋巴管生成和成熟个体淋巴管生理功能的调节因子,它仅在正常成人呼吸道和消化道的内分泌细胞以及内分泌腺细胞有少量表达。②在肿瘤组织中可见肿瘤周边新生淋巴管、扩张的淋巴管、有癌栓的淋巴管和有肿瘤转移淋巴结内淋巴管内皮细胞血管内皮生长因子受体3呈高表达。③血管内皮生长因子C/血管内皮生长因子受体3调控系统可调节肿瘤间质血管网的形成和淋巴管的生成。结论:①血管内皮生长因子C和(或)血管内皮生长因子受体3在结直肠癌、前列腺癌、胃癌、食道癌、乳腺癌等肿瘤细胞表达均增高,且与肿瘤细胞的转移关系密切。②一些特异作用于淋巴管内皮的生长因子及受体(如血管内皮生长因子C和血管内皮生长因子受体3)为研究淋巴转移开辟了新途径。
BACKGROUND: There are many researches indicating that vascular endothelial growth factor-C (VEGF-C) is closely associated with lymph node metastasis of malignant tumor. VEGF-C can selectively induce the proliferation of lymph vessel, and regulate angiogenesis.
OBJECTIVE: To review the structure, distribution, its receptor, function, and possible mechanism of VEGF-C in lymph node metastasis of tumor, so as to find the factors for the growth and metastasis of tumor cell.
RETRIEVAL STRATEGY: A computer-based online search of Medline and PubMed was undertaken to identify the articles about VEGF-C published in English from January 1995 to December 2004. Meanwhile, we searched CBMdisc and China Periodical Full-text Database for Chinese articles dated from January 1995 to December 2004. In addition, we manually searched 405 articles that were published. Articles that with ①human tumor organs as subjects with samples ≥30; and ②published recently among the similar contents were selected. Animal researches, repetitive research or poor quality were excluded.
LITERATURE EVALUATION: Thirty-two articles were accorded with the inclusive criteria including review articles, case reports and special topics.
DATA SYNTHESIS: ①So far VEGF-C and its receptor (VEGFR-3) is the only group of regulatory factor that regulates the formation of lymph vessel of embryonic tissue and physiological function of lymph vessel in mature individual. It is merely expressed in the endocrine cell and endocrine glands cell of respiratory tract and alimentary tract of normal adults. ②VEGFR-3 presents high expression in the tumor tissue with new lymph-vessel and dilated lymph-vessel around tumor and the tumor tissue on lymph-vessel with carcinoma embolus and lymph-vessel endotheliocyte in lymph node with tumor metastasis. ③VEGF-C/VEGFR-3 regulatory system may regulate the formation of tumor mesenchymal vascular net and the genesis of lymph vessel.
CONCLUSION: ①VEGF-C and (or) VEGFR-3 are (is) highly expressed in tumor cells of carcinoma of colon, cancer of rectum, cancer of prostate gland, gastric cancer, cancer of esophagus, mammary cancer, and so on, and it is closely associated with the metastasis of tumor cell. ②Some growth factors and receptors with specific effect on endothelium of lymph-vessel such as VEGF-C and VEGFR-3 open a new path for studying lymphatic metastasis.
